1.The correctionslisted below will require a Right of way permit(please find attached a Right of
Way permit application). A traffic control plan will need to be submitted with the ROW
application.
2.Asphalt joints need to be re-sealed for the 2’ asphalt cutsand patches (midway frontage and
th
across 84Ave West).
3.Asphalt crackingat the north side of the frontage improvements within the right of way where
the 2’ asphalt cut ends will need to be sealed.
4.Due to joint tack placement,striping will need to be redone(fogline and center lines).
5.A section of the concrete approach curb/gutter will need to be replaced. Please see image below
forlimits to be replaced within red.Inspections will need to be scheduled with the obtained
Right of Way permit.Concrete removal shall have a clean saw cut along all 4 sides of the gutter.
Inspector will inspect cuts to determine approval for concrete pour.
6.Per Olympic ViewWater & Sewer District (OVWSD), the sewer cleanouts for the site at the
right of way will need to be brought to grade and set to specification. Please see attached detail
drawing S-3.

7.Asphalt walkway has settled around the water main valves. The section of asphalt walkway will
need to be repaved for a consistent slope.Inspections for replacement to be scheduled through
right of way permit.
The actions or corrections indicated above are required to be corrected
prior to Maintenance Bond release.
