Construction can begin, subject to construction permitting from the local building official. Project Close-out Requirements Once the below have been completed, we will notify Department of Social & Health Services (DSHS) that you have completed the review process and are ready for final inspection Please note the following: The stamped “Authorization to Begin Construction” drawings will be delivered to the Facility Administrator. These shall be kept available on site for inspection during construction and at post occupancy survey. Final licensing approval is subject to a site inspection by DSHS and the Office of the State Fire Marshal to verify compliance with licensing regulations and your approved documents. If stamped approved plans are not available, the site inspection may be terminated until such time they are available as determined by the licensing inspector/surveyor. Any changes/deviations (incl. change orders or addenda) from the stamped documents must be submitted to the Department for review and approval. Please include your CRS number on all communications to Construction Review Services. Also, both 20 min. doors 67 & 68 must positively latch (via auto closers) when door is released from full open position. –kas Approved 7/24/15 – based on documents received 7-16-15, Door 67 shall be 20 min rated (see attached revised door schedule dated 7-14-15). Not Approved 6/24/15 – based on documents received 5-28-15, this comment is approved except for the following; these wall heaters need to shown to mount as high as possible (suggest contacting product mfr for distance from ceiling requirements) to avoid burning these special Memory Care residents. The Paw-guard option may still be necessary. Consult with the Facility Administrator and the manufacturer, then contact your CRS reviewer with the results of those discussions prior to proceeding with this issue. Also, indicate the finished ceiling height in this bedroom. -kas Approved 7/24/15 – based on documents received 7-16-15, in lieu of the King wall heaters specified 5-26-15 we would like to instead use the Ouellet ORC Sunshine Radiant Cove Heaters which can be installed on the wall 4.5” down from the ceiling (see attached spec sheets). One (1) 450watt heater in each sleeping area as shown on plan. Ceilings are 8 feet high. -kas 9 Provide catalog cuts for all new door hardware. Not Approved 6/24/15 – based on documents received 5-28-15, CRS reviewer is consulting with DSHS directly, however, Lobby 50 does not meet the requirements of WAC 388-78A-3050. Contact your CRS reviewer to discuss, what, if any, options exist for compliance with this regulation. -kas Approved 7/24/15 – based on documents received 7-16-15, the entire facility is memory care and residents can move freely throughout the building. -kas 11 Indicate what corridor wall ratings are intended. Approved 6/24/15 – based on documents received 5-28-15. –kas 12 Indicate what is provided and intended for window types at existing and new windows in this project (fixed, sliders, size of openings, screens, etc). The openings are also located within the 44” maximum distance to the floor. -kas 13 Indicate what room furnishings and accessories will be provided in the new resident room to fully comply with WAC 388-78A (2 towel bars, 2 wardrobes of what size, the correct number of electrical outlets, etc). Approved 6/24/15 – based on documents received 5-28-15. Refer to added comments 17 & 18 below. –kas 14 Indicate where Nurse Call devices are located. Approved 6/24/15 – based on documents received 5-28-15. –kas 15 Provide ventilation to comply with 2012 IMC outdoor air requirements. Approved 6/24/15 – based on documents received 5-28-15. –kas 16 Indicate how you will separate work areas from other residents and maintain work areas with a negative pressure space. Also, do not allow dust/ debris to enter existing exhaust and/or return air ducts. Approved 6/24/15 – based on documents received 5-28-15. –kas 17 Indicate where secure (lockable) storage for each resident is located (indicate how the location/unit complies with WAC 388-78A-3010(1)(i) Approved 7/24/15 – based on documents received 7-16-15, each resident shall have a night stand with a lockable drawer meeting the requirements of WAC 388-78A- 3010(1)(i). -kas 18 Indicate where additional storage for each resident is located (indicate how the location/unit complies with WAC 388-78A-3010(1)(j) Approved 7/24/15 – based on documents received 7-16-15, each resident shall have a wardrobe for adequate storage of clothing and personal possessions per WAC 388-78A-3010(1)(j). -kas 19 Patrick Shepard requested the following in an email dated 7/6/15; The wall & door separating the corridor (and this small renovation) from the rest of the community appears to be smoke/fire door. We would if possible like to do away with this door. Scarlett at 360.236.2949 to schedule a Final Inspection within 2 weeks of completing construction efforts. Compliance with the comments above provided by the Department of Health, Construction Review Services, are necessary for this facility to meet the requirements of the applicable licensing regulations found in the Washington State Administrative Code and associated references. These comments do not relieve the facility from the responsibility to meet the requirements of any other applicable federal, state or local regulations.
