This project is for use or licensure. The construction documents have been reviewed and
construction can begin, subject to construction permitting from the local building official.

Please note the following:
The stamped gs will be delivered to the Facility
Administrator. These shall be kept available on site for inspection during construction and at post
occupancy survey.
Final licensing approval is subject to a site inspection by DSHS and the Office of the State Fire
Marshal to verify compliance with licensing regulations and your approved documents. If stamped
approved plans are not available, the site inspection may be terminated until such time they are
available as determined by the licensing inspector/surveyor.
Any changes/deviations (incl. change orders or addenda) from the stamped documents must be
submitted to the Department for review and approval.
Please include your CRS number on all communications to Construction Review Services.

Not Approved 7/24/13 A nurse call device is required at EACH BEDSIDE so that
it is convenient to the resident. The drawings received 6/28/13 show a nurse call at
the south wall of the unit and does not meet the intent of the code. MZ
4 Verify that wall heater and existing HVAC Supply & Return air in room 52 have
capacity to adequately heat this room (able to maintain 70°F. WAC 388-78A-2990(1)
Not Approved 7/24/13 Per response received 6/28/13, the thermostat for the
central system is located in the corridor and additional wall heaters are provide in
the sleeping room and bathroom.
The supply and return outlets/inlets must be adjusted so that the air supplying this
room is comfortable as a resident sleeping unit. The central system cannot supply

air that is too cold or too hot. Provide a balance report and a statement to address
these concerns. MZ
5 Indicate how the residents in room 52 have control over the HVAC Supply air into this
room. WAC 388-78A-2990(5)
Approved 7/24/13 Per response received 6/28/13, the residents will be able to
control the supply air from the thermostat in the corridor. The residents can adjust
the temperature of the room with the wall heaters. MZ
6 Verify if room 52 has a cooling capacity via the HVAC system. WAC 388-78A-2990(2)

7 Indicate what the use of space 53A is intended for.

Compliance with the comments above provided by the Department of Health Construction Review Services are necessary for this facility to meet
the requirements of the applicable licensing regulations found in the Washington State Administrative Code and associated references. These
comments do not relieve the facility from the responsibility to meet the requirements of any other applicable federal, state or local regulations.
In the event of conflicts between other jurisdictions and these written comments, the most stringent shall apply.
