This project is for use or licensure and construction is to begin.
Authorization to begin construction may be issued once significant issues with the construction documents
have been resolved. The project will not be approved until the comments labeled as not approved have
been resolved by providing a written response to each of the comments.
When referencing drawings or attached materials, provide sheet or page #, revision # and date as applicable.
Additional comments may follow based on design team response and site inspections may be required to
verify compliance. All documents submitted will be retained in our office until all the construction
documents have been received, reviewed, and accepted. When the project is authorized to begin construction,
a complete package of accepted documents will be stamped and signed by the department. One copy will be
returned to the facility administrator to be kept on-site for the duration of construction and one copy will be
retained for our records.

Plan Review Comments:
1 Provide two sets of Fire Alarm plans as reviewed and stamped Approved by the local
AHJ (City of Edmonds). Drawings must bear the City Approval Stamp including
signature and date of review Approval. Upon review of this stamped Approved set of
plans this comment will be marked Deemed (effectively same as Approved). The
submittal shall include all product data cut sheets and applicable calculations for a
complete submittal. 2012 IFC 907.1
2 Provide two sets of Fire Sprinkler plans as reviewed and stamped Approved by the
local AHJ (City of Edmonds). Drawings must bear the City Approval Stamp including

3 Verify that egress path is operationally maintained from Both Main Dining Rooms
travelling North thru Life Station rooms. Also, that both sets of doors remain unlocked
and operational (actually are not capable of locking) from the Dining Room side at all
times. 2012 IBC 1014.2
4 The Occupancies on the ground floor are considered R-2 & S-2. Provide 1hr separation
between the two occupancies. Revise documents accordingly including details and
callouts for penetrations and opening protections. 2012 IBC Table 508.4
5 Provide 60min door assembly (garage side) of the Elevator 135. 2012 IBC Table 716.5
6 Verify that this Facility is NOT intending to operate with contracted residential care
services. WAC 388-110
7 Provide product data sheets for all Fireplaces which demonstrate that all units are low
temp and will not be hot to the touch or capable of burning residents, staff or visitors.

8 Verify that the courtyard doors will remain unlocked at all times for resident use unless
outside conditions present a danger to the resident such as icy (slip/fall), excessive heat,
lightning, or similar conditions. WAC 388-78A-2380 (9) (a)
9 Verify roll in showers have a threshold that is ½ maximum.WAC 388-78A-3030(8)(a)

10 Submit communication system (nurse call) shop drawings that identify a means for
staff and residents to summon on-duty emergency assistance, in all toilet rooms,
corridors, common areas accessible to residents, bathrooms, resident living rooms and
sleeping rooms. WAC 388-78A-2930
11 Revise E4.1 and other applicable drawings to indicate that (2) telephone lines are
provided at each phone jack in each resident room. WAC 388-78A-2930(3)
12 Provide location on site plan and details for the grease interceptor.
13 N/A Recommend bollards to protect from vehicular damage at drive side of transformer and
just inside gate (center) at generator.
14 Provide details of grease exhaust duct indicating required rating. Use NFPA 96, and
Sections 506 and 507, of the International Mechanical Code, as a checklist when
preparing detail drawings. Include at a minimum the following; grease duct size and
gauge, clearances between the duct and shaft (or if there is a proposed duct wrap
rated) between fan outlet and the roof and the top of the shaft and the roof. Show
locations of cleanouts in the shaft and duct access openings. Include catalog cut sheets
and specifications for any rated duct wrap product. WAC 388-78A-2910 and
2850(1)(b), and Section 610, International Fire Code
15 Provide information to identify the required backup source of heat in enough common
areas to keep all residents adequately warm during interruptions of normal heating
operations. WAC 388-78A-2990 (3)
16 Revise A5.0 and related Mechanical drawings. The exhaust flue for furnaces F5 & F7
appear too close to the intake for RTU-1 (10ft min separation). 2012 IMC 401.4
17 Describe in the functional program the policy and procedures for ensuring that
poisonous or toxic plants are inaccessible to residents who, based on their diagnosed
condition or cognitive disabilities, may ingest or have harmful contact with such plants.

18 Provide specification for curtains, shades or blinds provided for visual privacy in each
room. WAC 388-78A-3010(1)(e)(ii)
19 Identify all doors with special locking arrangements (such as keypad with code for
egress or similar) and provide selected product information and narrative. All doors are
required to meet 2012 IBC Section 1008 and WAC 388-78A-2380.

21 Provide exit signs on sheet E2.3; one at SE corner of Corridor 230 with arrow pointing
North & one above North door (room side) of Life Station 260. 2012 IBC 1011.1
22 Remove exit sign above door to Lobby 100 (garage side) or keep this door unlocked at
all times. 2012 IBC 1014.2 (2)
23 Resolve the dead end corridor at the South end of Lobby 100 (50ft max. to East door
exit). Conference 108 doors can move North. 2012 IBC 1018.4 (Exception 2)
24 Provide an exit at the SW end of garage. Two exits are required from this South portion
of the garage (more than 29 occupants). Suggest adding a door where sheet E2.1 has
indicated an exit sign. 2012 IBC Table 1021.2 (2)
25 Include the two Storage rooms 109 & 124 in the S-2 Occupancy. 2012 IBC Table 508.4
26 Provide an explanation of what is really intended for the function of rooms 243 & 343.
Suggest changing the name of rooms 243 & 343 from Assembly to something else.
27 Provide an indication (explain or identify how for all 6 sub-sections of this State
Amendment) of how rooms 243 & 343 comply with 2012 IBC 1018.6 (Exception 2).
28 Provide catalog cuts for all sinks/showers used, including faucets. WAC 388-78A-3030
29 Provide an AVB (usually at wall plumbing connection to hose) at all shower hoses and
any hose/sink locations. 2012 UPC 602.3 & 603.3.2
30 Provide backflow protection at water supply to hard plumbed devices such as ice
machines & beverage machines. Also provide model number of main building
Backflow protection device. 2012 UPC 602.2 & 603.3.7 and WAC 388-78A-2950 (2)
31 Provide product cuts on all washing machines used. WAC 388-78A-3040 (3)
32 Provide product cuts on all on the dishwasher used. WAC 246-215-04256 (1)
33 Provide a Type II hood over the dishwasher. 2012 IMC 507.2.2
Additional comments may arise upon CRS review of subsequent submittal materials.
Compliance with the comments above provided by the Department of Health, Construction Review Services, are necessary for this facility to meet
the requirements of the applicable licensing regulations found in the Washington State Administrative Code and associated references. These
comments do not relieve the facility from the responsibility to meet the requirements of any other applicable federal, state or local regulations.
In the event of conflicts between other jurisdictions and these written comments, the most stringent shall apply.
