The City has agreed to issue the requested permit for PSE's Cross Bore Program allowing for Hydromax to TV the City's
sewer laterals/mains. Please find attached, the requested permit ENG20190095. Consistent with the previously issued
permit (ENG20140445), if additional review and/or inspection time is needed the City may charge additional review fees
and/or inspection fees. Below are additional requirements established with issuance of the initial permit.
• The City would like Hydromax to notify the following people, via email, the area they are working at a
minimum of 24hrs prior. If more time can be provided, please email sooner.

2) Chris Rivera — chris.rivera@edmondswa.gov
3) Jim Waite — jim.waite@edmondswa.gov
4) Tim Harris—tim.harris@edmondswa.gov

6) Megan Luttrell — megan.IuttrelI@edmondswa.gov
Attached is a copy of a map that shows where the City requires site specific traffic control plans. (The
green lines and hatched areas).
1) A traffic control plan for those designated areas will be required for review and approval prior to
any work being completed.
2) A inspection fee will be added to the permit and an inspection is required.
3) Please note that any area where there is a need for a police officer, the police officer must be a
City of Edmonds Officer. To request the officer, contact Jason Shier at 206-353-2987, 10 days
prior to the start of work.
4) Please also note that any work on Edmonds Way/SR-104, 196th St SW/SR-524 and Highway 99
will require a lighted arrow board.
• If any repairs need to be completed on the sewer lateral and/or main line, a separate permit will need to
be obtained by the contractor completing the work. All repairs will need to be inspected by the City of
Edmonds. If all repairs will be completed by 1 contractor, we can issue one permit for the life of the
Hydromax permit. The City would add additional inspection fees as needed for each repair.

Hi Chris,
The link sent does contain a lot of data from the last 4 years. It may take a while to download and view. This is the
format that Hydromax USA uses to send their annual deliverables. Prior to a sharefile link methood, thumb drives were
sent out with the same data. Please let me know if you are able to pull information from this link. Thank you.

Teagan,
Thanks for looking into the video files and sending them over. I forwarded the sharefile link to our Utilities Engineer,
Mike DeLilla. Though there was comment regarding whether or not we will be able to open the files. It was stated that it
was mentioned back in 2014 that the video was supposed to be in a format that we can open and read. At this time
myself and Mike are trying to open the files and make sure that we can pull information. Will keep you informed.

Thank you for speaking with me this morning. Below is the sharefile link for the City of Edmonds CCTV videos dated back
from 2014 to 2018 for the exchange of our 2019 Blanket renewal permit. From now going forward, the City of Edmonds
is on the annual deliverable list and will receive a sharefile link the beginning of each Calendar year in exchange for our
Blanket renewal. I do apologize for the miscommunication from the years prior. Please let me know if there is anything

I'm currently in the process of uploading the deliverables for Edmonds, including videos from 2014 to 2018.
