During review of the above noted application, it was found that the following information, corrections, or
clarifications are needed. Please submit revised plans/documents including a written response to each item
below.
Resubmittals can be made at the Development Services Department on the 2nd floor of City Hall. Permit
Center hours are M, T, Th & F from 8am-4:30pm and on Wednesdays from 8:30am-12pm.
City of Edmonds handouts, New updated standard details with new reference numbers, application
and other forms, and development code may be referenced on the City website:
www.edmondswa.Eov under Services/Permits and Development.

2. 2/5/19 - Please submit a Street Use Agreement for the sign on the north side of the building, currently
above the city's sidewalk/right of way adjacent to 220th St Southwest. Please refer to the handout
E#75 and attached Street Use Agreement. The Architectural Design Board Approval in not required.
2/5/19 - The previously provided Certificate of Insurance is not consistent with the city provide
example for a Street Use Agreement. Please provide an update certificate that is consistent with the
example provided, please provide the following.
a. A Certificate of Insurance accounting for Personal Injury and Property Damage up to
$300,000 (please see example).

b. The certificate of insurance will need to list the City of Edmonds as an additional
insured such as, " It is understood and agreed the City of Edmonds is named as an
additional insured for the sign located at 22001 Highway in the City right of way"
(please see example).
c. City of Edmonds will need to be listed as Certificate Holder (please see example).
