Heinzig Enterprises 169093 d Ave SE Mill Creek, WA 98012 RE: Request for Alternate Design for Calais Apartments 11154 th Ave S, Edmonds, WA Plan Check # 2006-1240 VIA FACSIMILE Dear Mr. Heinzig, The City is in receipt of your request for Alternate Design pursuant to IBC 104.11. The Alternate Design request is approved for a limited scope voluntary seismic assessment and seismic rehabilitation of the subject building based on a combination of UBC/ASCE 31-03 (Tier 1) seismic assessment criteria, and 1982 UBC/IBC 2003 seismic design criteria with the following conditions, exceptions, and clarifications (following the formatting of the request): • Section V: Alter matrix to reflect conditions of approval below, as well as outstanding plan review comments as applicable. • Section VI iii: Shop Drawings (with engineer review approval stamp) must be submitted to the City Building Division prior to final framing approval by the City Building Inspector. For clarification, recognizing that framing for this project will be phased and partial framing inspections will be conducted, final framing approval will be granted by the City Building Inspector once all framing for the project is complete. • Section VII iii: As -Built Drawings (see City response to Section X)- • Section VII iii: Structural Observation Report must be submitted to the City prior to final framing approval by the City Building Inspector. • Section IX a: o Based on the proposed Shear Wall Schedule on Sheet SLI of the plans submitted as part of the permit application, Special Inspection will be required for Types 4, 4b, and 5 shear walls, and the City Building Inspector will inspect Types 1, 2, and 3 shear walls. ' Incorporated August 11, 1890 • Sister City - Hekinan, Japan Page Two March 28, 2007 o Periodic (not continuous) special inspection is permitted per IBC 1707.3 for ".._nailing, bolting, anchoring and other fastening of components..." o The City agrees to perform a daily afternoon inspection between the hours of 1:00 pm and 3:00 pm (not to exceed one hour). If the City is unable to provide the daily inspection construction shall remain exposed for City Building Inspector approval. Inspection by the Special Inspector (E.O.R.) cannot substitute for required City building inspections. If on any day the City Building Inspector arrives for the agreed upon inspection and work is not ready for inspection, a $50 reinspection fee shall be assessed and must be paid within 48 hours. To avoid a reinspection fee a cancellation request must be received between 9:OOam to 10:00am by calling 425-771-0220. o Installation of %" rigid insulation in exposed exterior wall cavities is acceptable and shall be available for immediate installation after the City Building Inspector has inspected the open cavity. The Building Inspector will then verify installation of the rigid insulation. • Section IX b: o Item 3: As noted in IX a above, based on the proposed Shear Wall Schedule on Sheet S 1.1 of the plans submitted as part of the permit application, Special Inspection will be required for Types 4, 4b, and 5 shear walls, and the City Building Inspector will inspect Types 1, 2, and 3 shear walls. o Items 9 & 10: Special Inspection Reports must be submitted to the City on a weekly basis, with a final summary Special Inspection Report submitted prior to final inspection approval. o ii Item 5 (Geotech Special Inspection Reports): Special Inspection Reports must be submitted to the City on a weekly basis, with a final summary Special Inspection Report submitted prior to final inspection approval. • Section X b i & ii: Copies of the noted P.E. stamped engineered repair details or `as-builts' (& supporting talcs if necessary), must be submitted to the City Building Division prior to final framing approval by the City Building Inspector. • Section X c iv: Copies of the noted P.E. stamped engineered repair details or `as-builts' (& supporting talcs if necessary), must be submitted to the City Building Division prior to final framing approval by the City Building Inspector. • Section XI a: Structural and Nonstructural inspections are required by the City Building Inspector in accordance with IBC 109. • Section XI b: Building Envelope inspections required by RCW 64.55 (EHB 1848) are in addition to those conducted by the City Building Inspector and do not substitute for required City building inspections (IBC 109). Type and frequency of inspections for the Building Envelope are determined by the Building Envelope Inspector per RCW 64.55. • Section X: o Simpson SET epoxy system or Hilti HIT HY-150 may be used for adhesive anchors since products are not yet available on the market that comply with AC308. Page Three March 28, 2007 o Continuous inspection of the epoxy anchor installation is required to insure proper installation.
