Al,
Prior to Final Sign-off the below listed items will need to be completed.
1. Sidewalk needs to be replaced.
2. Trenches need to be backfilled.
3. There should be no exposed soils (area should be landscaped).
4. Excess materials and spoils should be removed from the site.
5. Asphalt parking and extruded curb will need to be replaced.
6. An as-built will need to be submitted.
Please call for an inspection once the sidewalk formwork is in place. If there are any questions,
please feel free to contact us.

Joanne,
I just received confirmation that Smith Fire will test the backflow on Thursday. I have already
removed the sediment sock and cleaned out the structure. This existing structure does not have a
base in it. You may want to wait for the inspection until after the backflow is tested.

Al,
I need you to take the sediment sock out of the catch basin before finalizing the project. We will
inspect the connection mudding and that there is no debris in the basin.

To all involved,
I am requesting the water to be shut down on Wednesday 4/15/15 at 8:00am to 10:00 am.
Recap of our meeting onsite: Scott is going to give 48 hour notices to all the businesses effected by
the shut down in his complex and hand deliver the confirmed signatures to Joanne COE. The COE
Water department will shut the water main down at 8:00am. Barry Civil will remove the existing
vault and single check. Install a 8” valve supplied by the COE water at the property side of the
sidewalk. Turn the water back on. We will finish connecting the new system and begin the flushing
at 12:00. Hopefully we will have time to hydrostatic test after the flush. If not we will hydrostatic
test on Thursday at 8:00 am.
