The attached site plan has been revised to indicate the power line crossing from the west side of 5 to
the east side. It looks like we will have enough clearance for the proposed monitoring well at that
location. I also slightly moved a proposed monitoring well at the northeast corner of the intersection of

5 Avenue and Maple Street to avoid a water line in that area. We will also be completing a private
utility locate before the drilling starts for additional confirmation that the proposed drilling locations will
be clear from underground utilities.

I received your phone message … you have great timing! I had your project pulled for review
today. Everything looks good except the power system isn’t shown completely (there is a crossing

from the west side of 5 to the east side that feeds the Hazel Miller Plaza). I’m a little concerned
that the proposed well location falls in line with that crossing. Can you double check the locates
(or call again if needed) to confirm location of this crossing.
I also have a message out to our Parks Manager for information on the storm connection from the

Plaza to the CB at the corner of 5 and Maple. I need to make sure that that well isn’t in conflict
with the storm line.
Once those two pieces come together we should be good for permit issuance. I’ll let you know the
permit fee at that time.

I have attached a revised version of our updated site plan which indicates the locations of the proposed
monitoring wells. My response to your questions is as follows:
1)See attached site plan which includes a detailed diagram of the well location and extent of
disruption.
a.Yes, the wells will be in the pavement section just outside the curb/gutter.

b.The well will be in the sidewalk at the intersection of 5 & Maple.

c.All of the proposed wells near 5 & Maple will be located within the sidewalk area and
not in the landscape area.

2)The stormwater mains and catch basins along 5 Avenue have been included on the site plan
diagram.
3)A detail indicating the construction of the monitoring well monument cap and concrete seal
(including dimensions) has been included on the site plan diagram.
4)The monitoring wells will be sampled on a quarterly basis (once every three months). A
passenger vehicle (i.e., support vehicle) will be parked along the street in the area adjacent to
where the monitoring wells will be located. A small peristaltic pump will be used to collect the
water sample from the wells. The sampling equipment and personnel will require an area
approximately three (3) feet by three (3) feet area around each well to complete the sampling
activities.

Good morning John,
Please provide the following information/clarification regarding the ROW construction permit to
install 6 groundwater monitoring wells in the vicinity of Old Milltown:
1)Please provide additional detail at each of the boring locations to indicate the extent of
disruption and exact placement of the well.
a.At street locations, will the wells be in the pavement section just outside the
curb/gutter?

b.At the intersection of 5 & Maple will the well be in the sidewalk ramp or in the
sidewalk?

c.Where in the landscape area near 5 & Maple do you propose well installation?
The City just completed the Hazel Miller Plaza and exact location will need to be
approved and then installation coordinated with the City Parks Department (I will
assist with this).
