The construction documents have been reviewed and found acceptable. Construction can begin, subject to
construction permitting from the local building official.
Project Close-out Requirements
Once the below have been completed, we will notify Department of Social
& Health Services (DSHS) Boarding Home licensing that you have completed the review process and are
ready for final inspection
Please note the following:
The stamped “Authorization to Begin Construction” drawings will be delivered to the Facility
Administrator. These shall be kept available on site for inspection during construction and at post
occupancy survey.
Final licensing approval is subject to a site inspection by DSHS Boarding Home Licensing and the
Office of the State Fire Marshal to verify compliance with Boarding Home regulations and your
approved documents. If stamped approved plans are not available, the site inspection may be
terminated until such time they are available as determined by the licensing inspector/surveyor.
Any changes/deviations (incl. change orders or addenda) from the stamped documents must be
submitted to the Department for review and approval.
Please include your CRS number on all communications to Construction Review Services.
PROJECT CLOSE-OUT REQUIREMENTS

1 What is the construction type at this facility? Is a sprinkler system installed? This
information was omitted from the application.
Approved 2/23/12 – Per response received 2/7/12, the building is of type VB
construction and a sprinkler system is installed throughout.
2 Provide fire alarm and fire sprinkler drawings for this project, if revisions to the existing
system are being made. International Fire Code Section 903.1 and 907.1
Approved 2/23/12 – Per response, no revisions to the sprinkler layout are being
made. A fire alarm strobe will be located in the sleeping room and bathroom per
revised plan received 2/24/12
3 Provide a nurse call system that will provide residents and staff with a means to summon
on-duty staff assistance from resident unit. WAC 388-78A-2930(1)(a)(i) Will a two-
way intercom system between staff and residents be provided?

be located in the unit near the door and in the bathroom.
4 Provide two telephone lines in the resident unit. WAC 388-78A-2930(3)

6 Provide a plan showing the individual temperature control serving each sleeping room
and living room. This must be located between 30-48” above the floor capable of
maintaining room temperature between 60-85 degrees. WAC 388-78A-2990

temperate air to the room and all mounted heater units will provide thermostat
controlled heated air to the units.
7 Provide a furniture plan showing the layout of the unit including bed locations and any
living room furniture (e.g. chairs and tables). Each sleeping room must meet the size
requirements of WAC 388-78A-3010(1)

8 Identify where a lockable storage space such as a drawer, cupboard or other secure space,
measuring at least one-half cubic foot with a minimum dimension of four inches, will be

9 Show how and where each resident will be provided a separate space for storage of
clothing and other personal items located in or immediately adjacent to the resident’s

11 Will the bathroom be attached to the unit? If so, provide plan that shows a door leading
to this unit. Provide an individual towel and washcloth rack or equivalent in the sleeping
room, unless the bathroom is attached and private to the resident sleeping or living room,
the individual towel and washcloth rack may be located in the attached private bathroom;

The bathroom appears large and possibly meets the requirement for a roll-in type shower.
If this bathroom becomes private and used solely by this unit, does that affect the
availability of a roll-in shower used by other units on the floor? WAC 388-78A-3030(8)

plans and response received 2/7/12.
12 Provide information on the existing toilets to confirm it is suitable for use as a resident
toilet room. Photos would be acceptable.
Review the requirements of WAC 388-78A-3030:
-Grab bars
-Exhaust fan
-Nurse call
Approved 2/23/12 – Per plans and response received 2/7/12, the existing bathroom is
approvable for use by the residents.
13 Does the renovation of the family room into a private room affect the facility’s total
available square footage of day room areas for residents? 20 sf is required per resident.
Provide calculation for required day room sf and remaining sf with the subtraction of the
space. WAC 388-78A-3050
Approved 2/23/12 – Per response received 2/24/12, the facility is required to have
680 sf of day room area (for 34 residents). The renovation of the family room
reduces the total sf of day room to 1929 sf.
14 Will this room be part of assisted living service contract with DSHS? If so, the physical
requirements of WAC 388-110 will apply.
Approved 2/23/12 – Per response received 2/7/12, the room will not be part of a
contract with DSHS for assisted living service.
15 Provide information on the doors in the corridor. Corridor walls are required to have a
30-minute rating and doors are required to be 20-minute.

have a 20 minute rating.
16 Note: Revised drawings were received 4/23/12, deleting the lavatory located in the
room is approvable. The facility is required to provide (2) lockable drawers within
the unit for the residents prior to occupancy.
Compliance with the comments above provided by the Department of Health, Construction Review Services are necessary for this facility to meet
the requirements of the applicable licensing regulations found in the Washington State Administrative Code and associated references. These
comments do not relieve the facility from the responsibility to meet the requirements of any other applicable federal, state or local regulations.
In the event of conflicts between other jurisdictions and these written comments, the most stringent shall apply.
