Arrived onsite per contractors request to observe the placing of concrete for Columns 6F.6,
7F.6, 6G.8, 611.8 and 711.8 Level 1 to 2. Observed contractor placing 5 c.y. of mix# 15LCD10. A
slump and temperature test was performed and 1 set of compressive strength cylinders were made.
See Report# 09617054CR222018JLG-1 for break results.
Per contractors request observed the stressing of tendons for PT Deck Level 1. The field cure
cylinders were picked up and taken to the lab. The cylinders were broken with the results meeting or
exceeding the required 3000psi to stress. Per contractor the BLU-BRN banded tendons (12 @ 118')
were not to be stressed due to a void behind the anchors on the live end. The tendons were stressed per
plans and specs until one of the YEL — BRN banded tendons (12 @ 111') caused the concrete to break
up and expose the anchors at the dead end. The 4 tendons starting from the east were stressed when
the concrete broke up. The tendon was at approximately 3000psi when the concrete broke up.
Stressing was stopped at approximately 1:00pm and contractor and Engineer of Record were
contacted. Contractor elected to end stressing for the day at approximately 2:30pm when no answer
was received from the Engineer of Record. Contractor will inform Krazan when the Engineer of
Record informs them of a fix.
