NOTES J DISCREPANCIES 1 00 % COMPLETE
Arrived onsite as per client —request. Confirmed welding personnel qualification and use of proper filler metal. Inspected the
natcrials to be used for the field welding and adhesive anchoring. The re -bar onsite was not ASTM A706-weldable and
here was not a sufficient amount of epoxy onsite to complete the anchor installations. Departed site while the contractor
;athered the proper materials. Re -arrived onsitc. Performed visual inspection of the re -bar attachment welds to the 2" micro
Tiles. As per a phone conversation with the EOR, the bar was to be tack -welded only. The correct grade, size, number, and
dimensions of re -bar was verified. Each micro pile received (3) 8x6" #4 re -bars, as -detailed. Each bar was welded with two
1" long vertical welds to the O.D. of the pile. Adhesive anchor installations were witnessed as per detail # 4 of S2.1. The
:adhesive used was Simpson Set XP (expiration 8-24-19). Minimum embed depth of 6" was verified. Pre -cleaning and
deposition of the epoxy was performed in accordance with the manufacturers printed installation instructions.
