Willowdale inspection, 5-7-09 & 5-6-09.pdf thth Willowdale (208 & 76) Inspection on 5/7/09 (install storm): 1) The unmarked sewer lateral from 5/6/09 (see below) would potentially interfere with the required Type I basin in the plan. Ed Sibrel stated that to fit the Type I they could shift the placement to the south by 6" if possible or they could use an inlet basin if not. Contractor used the Type I and shifted (south) by approximately 6" 2) Pipe was bedded in pea gravel and per Rob English used crushed rock as backfill. – Geotech arrived on site as I was leaving to begin compaction tests. Discussion on 5/7/09: 1) Contractor asked to jackhammer trench for the fire line install. – Told contractor that a saw cut was required at this time as stated in our Standard Details. 2) Contractor had questions regarding purity tests and install of the line that the water service lines will be connected to. Spoke w/Kris Kuhnhausen and informed the contractor that we will not have answers for them until Jeanie McConnell comes back on 5/13/09. Inspection on 5/6/09 (Install sewer): 1) Mike Johnson gave approval for contractor to use a pvc coupling to reconnect the storm line that they needed to cut out in order to lay the sewer line. The storm line did not need to be cut out. 2) The new sewer line was not able to be placed at bench per plans due to a forceline connected to Lynnwood's manhole. This then changed the fall to 1%, due to the raise of the sewer line and other utilities in the trench. 3) Pipe was bedded in pea gravel and crushed rock was used a backfill. – Geotech was onsite taking compaction tests. 4) During trenching a sewer lateral was broken (lateral was not marked). Jim Clemons approved for the contractors to tie into the new 8" sewer line (wye - 22 - 45 - about a 1" of pipe and then fernco). The other end was plugged. Therefore the new 8" line will not be tested. The contractor was informed that Public Works will TV the line for approval. If the TV fails, the contractor was notified that they will be required to fix the sewer line per our standards. 5) Mike Johson gave permission to the contractor to reroute the conduit for the signal and pedestrian wires to allow for the sewer manhole to be placed per plans. 6) Kris Kuhnhausen and Jennifer Collins were on site during the install of two ecology blocks into trench behind the existing thrust block on the 90 degree water main bend – Kris approved installation Disscussion on 5/6/09: 1) Mike Johnson gave permission for the contractor to temporarily splice the loop for the lights. They are required to replace the loop with one line.
