STRUCTURAL OBSERVATION REPORT
Project: Mock Residence Slope Stabilization
Purpose: Structural Evaluation
Location: 21100 88th PI West
Date: August 12, 2019
CG Project: 18295.10
Report: 3
Client: Darrin Mock
General Contractor: Unknown
Field Rep: Morgan Kruse, PE
PURPOSE AND SCOPE
A field representative of CG Engineering was on site on August 5, 2019 to observe the structural slab
dowel bar installation to the existing foundation. Prior to our site visit, approximately 75'-0" total
length of concrete grade beam had been poured and completed, along with the block retaining wall
below the structural slab.
EXISTING BUILDING
The existing building is a single family residential structure that had experienced settlement to the
downhill side. The existing rockery wall at the edge of the rear patio was removed and will be replaced
with an allan block and geogrid wall system supported by a concrete grade beam. The concrete patio
slab will then be replaced with a new structural slab. At the time of our site visit, the entire length of
grade beam was completed and the block wall was installed.
To the best of our knowledge, the construction that we observed was in accordance with the approved
structural drawings with the exception of the items listed below:
OBSERVATIONS & RECOMMENDATIONS
1. Structural Slab Reinforcing — Prior to our arrival on site, the holes for the dowel bars were
drilled into the existing footing and cleaned as required for the epoxy installation. During our
site visit, the epoxy was inserted into the drilled holes, and the dowel bars were inserted. The
epoxy used was USP CIA -GEL 7000-C and Sika Pro Select. The slab dowel bars and epoxy
appeared to conform to the construction documents and no issues were noted.
