Engineer. Every flagger must be trained as required by WAC 296-155-305 and must have certification
verifying completion of the required training in their posession.
RESTORATION Restoration is to be in accordance with City codes. All street -cut trench work shall be patched with
asphalt or City approved material prior to the end of the workday - No Exceptions
WARRANTY The contractor is responsible for workmanship and materials for a period of one year following the final
inspection and acceptance of the work.

Good afternoon Natalie,
I have attached a revised planset and answers to your questions are listed below in blue.
• The site plan does not show all utilities in the High St ROW. In addition to the City water main, there is a City sewer
main and a City storm main. Please revise to show all utilities. All utilities are now included on the revised planset.
• The revised plans show a perpendicular crossing and a bore path on the east side of High St that is parallel to the
ROW. Please show that the minimum 5' horizontal and 1' vertical separation between the bore path/service and all
City utilities can be achieved. Power and Gas are shown on the east side of the ROW. Make sure that the proposed
bore path has at least 12" of horizontal separation from the gas main. Revised route will ensure a min of 5' horizontal
separation of all mains, and 1Oft horizontal of utility structures.
• Is the proposed flush monument in the ROW? If so, what are its dimensions? The 7"flush mount carson to be placed
on customer property line
• What is the tie in point? Is it an existing structure or will a new structure be placed? Please clarify. We will tie in at an
existing CATV PED as I have added a picture in the top right corner of the planset.
