Engineer. Every flagger must be trained as required by WAC 296-155-305 and must have certification
verifying completion of the required training in their posession.
RESTORATION Restoration is to be in accordance with City codes. All street -cut trench work shall be patched with
asphalt or City approved material prior to the end of the workday - No Exceptions
WARRANTY The contractor is responsible for workmanship and materials for a period of one year following the final
inspection and acceptance of the work.

2. All sidewalks, driveways, exits and egresses
shall be completely clear unless otherwise indicated.
3. Channelizing devices are 28" traffic cones.
Use traffic drums (barrels) with steady burning
Type C lights on all State State Routes and Hwys.

Hi a test lead wire is a wire that is attached to the test lead box on site for gas
reads they need to install all brand new wires for reads, for this box they are
getting low reads. The work is in concrete / grass there is no boring no
trenching just one cut 3x5. The work is located on the south side of Sprague
that will be where the one cut is to serve 916 & 919.
Requested Work: Dig up bond located at 110' ECL 9th Ave N &
19' SCL Sprague St. Cut out all old bond wires and install 4
new ones: two 10 gauge wires labeled #4 on the west side of the
insulated fitting and two 10 gauge wires labeled #2 on the east
side of the insulated fitting. Bond one #4 and one #2 wire
together with a split bold and bring all wires up in a T/L box.

Hi Sue,
Can you please clarify the scope of work for me for this permit? What is a test lead wire? Will there
only be a cut at the gas main on Sprague, no boring across Sprague? If there will be boring, please
submit a revised plan showing potholing for all City utilities in the ROW (e.g. city sewer and water
mains).
