Meadowdale Beach RD. Retaining Wall, Permit Number BLD2019-0935 Dear Mr. Buehler, As requested, we have completed our Earthwork Observation and Testing Services for the subject site located at 16219 N. Meadowdale Road in Edmonds, Washington. Based on City of Edmonds Special Inspection and Testing Agreement requirements, we were required to observe the following items during construction to verify the work was performed in accordance to the approved project plans and recommendations provided in the referenced Preliminary Geotechnical Report. 1. Soils Evaluation. 2. Retaining Wall Installation. 3. Retaining Wall Drainage Installation. At periodic intervals during the construction process, the project General Contractor contacted us to arrange site visits to observe and monitor the special inspection items listed above. At the time of each site visit, we prepared a Daily Field Memo delineating our observations, documenting our test results, and providing recommendations as necessary to assist in the successful completion of the project. Our site visits began on December 15, 2020 with the beginning of the wall construction. At the time of this letter, all of the Special Inspections items listed above have been completed. Mr. Brett Buehler January 4, 2021 SPECIAL INSPECTION ITEMS Soils Evaluation Prior to the planned retaining wall construction, we performed an evaluation of the soil and groundwater conditions at the site. We presented our findings in the referenced Preliminary Geotechnical Report. In the report, it was explained the retaining wall foundation subgrades were expected to be excavated down to native, medium dense, moist, sand with silt and gravel consistent with Advance Outwash. The soils we observed on site during construction of the retaining wall consisted of native, medium dense, moist sand with silt and gravel consistent with Advance Outwash. All foundation subgrades observed consisted of these native soils as expected. Retaining Wall Installation We began our observations of the segmental block wall construction on December 15, 2020. At the time of our site visit, the base of the wall alignment was excavated to a minimum depth of 0.5 feet below surface grades. The existing exposed soils along the wall alignment were found to consist of medium dense, moist sand with silt and gravel consistent with Advance Outwash as expected. Prior to wall construction, the contractor placed a leveling course consisting of 5/8 inch crushed rock. Another leveling course of concrete was then placed over the crushed rock. We returned to the site the following day on December 16, 2021 to continue our observations of the wall construction. At the time of our site visit, the contractor was continuing to construct the wall and began installing the drain system behind the wall. The wall drain will outfall at the low end and extend under the driveway where the channel drain is planned. The outfall will be the same as for the channel drain. We returned to the site the following day on December 17, 2021 to continue our observations of the wall construction. At the time of our site visit, the wall was nearing completion. The contractor began excavating the area where the quarry spall drain outlet would be. We returned to the site the following day on December 18, 2021 to continue our observations of the wall construction. At the time of our site visit, wall construction and drainage was complete. Based on our site observations, the retaining wall was constructed in accordance with the project specifications and Ages Engineering recommendations. Retaining Wall Drainage Installation We visited the site on December 16, 2020 to view the first installation of the subsurface drainage installation behind the wall on the site. At the time of our site visit, we observed a 4-inch perforated PVC pipe was placed behind the wall in all locations. We returned to the site on the following 2 days to continue our observations of the drainage placement. The pipe was covered with 1-inch washed drain rock to within 1.0 feet of the surface. At 1.0-feet below the ground surface, a layer of separation fabric was placed over the top of the drain rock. Organic -laced soil was then placed over the separation fabric. Page - 2 - Mr. Brett Buehler January 4, 2021 We returned to the site on December 26, 2020 to observe the final drainpipe discharge location into the quarry spalls. Based on our site observations, the subsurface drains were constructed in accordance with the project specifications and Ages Engineering recommendations. CONCLUSIONS In our opinion, based on our observations during construction, the special inspection items listed above were completed in accordance with the approved project plans and the recommendations provided in the referenced Preliminary Geotechnical Report and by Ages Engineering personnel in the field. At the time of our site visit, the CornerStone Block wall construction was ready to begin. The wall is to be Constructed of F200 ComerStone Blocks and capped. The base block excavation is 6" or more below grade. A gravel with minus course was laid down and compacted with a plate Compactor. A thin layer of concrete was placed on top of the crushed compacted rock and the base blocks placed On top of the concrete. We reviewed the site plans with the project manager. Questions around the drainage were raised. The Ages Engineering representative spoke with the geotechnical engineer and discussed the drainage options. Option one was To tie into the storm water drain; the other option was to create the Swale and drain the water on to quarry spalls. The Onsite project manager said he would get back with us on which option to install. At the end of the day the first layer of block had been laid. At the time of our site visit, the CornerStone Block wall construction was underway. Clean washed rock has Been delivered for use in the backfill. (River Rock). The wall drain was installed and connected to the outlet. We conferred with the Project Manager and it has been determined to drain the wall drain into the quarry spalls and Not tie into the storm drain. A discussion around how to install both the wall drain and the channel drain was had. The onsite contractor understood the wall drain would be placed under the channel drain and both pipes would drain Into the quarry spalls. At the end of the day one third of the wall has been built and backfilled. We will return again tomorrow to continue With building the wail. The wall has been completely backfilled with clean washed rock up to within V of the top of the wall. Filter fabric has been placed on top of the rock and covered with onsite soils. The retaining wall drain has been connected with a solid pipe and laid in under the grated driveway drain. The drive Way drain has been installed and both outfall to the Swale. At the end of the day the only item left is to fill the Swale with quarry spalls. The Project Manager said they would Take care of doing that in a few days. Retaining Wall Prior to issuance of a permit, this form must be completed in its entirety and returned to the City for approval. The completed form mast have signatures of acknowledgment by all parties. DUTIES AND RESPONSIBILITIES Special Inspection Firm And Special Inspectors: The Special Inspection firm of Ages Engineering, LLC will perform special inspection for the following types of work (separate forms must be submitted if more than one firm is to be employed): _Reinforced Concrete _Bolting in Concrete _Prestriessed Concrete _Shotcrete _Structural Masonry Structural Steel/Welding _High Strength Bolting _Spray applied Fireproofing _Smoke Control Systems _Lateral Wood Soils Evaluation _X_ Retaining Wall Installation _X_ Retaining Wall Drainage Installation _Other All individual inspectors to be employed on this project will be WABO certified for the type of work they are to inspect. If inspection is for work that is not covered by the WABO categories, or the inspector is not WABO certified, a detailed resume of the inspector and firm must he submitted_ The resume must show that the inspector and the firm are qualified by education and experience to perform the work and testing required by the project design and specifications. The work shall be inspected for conformance with the plans and specifications approved by the City. Revisions and addenda sheets will not be used for inspection, unless they have been approved by the City. Inspection records shall include: A daily record to be maintained on site, itemizing the inspections performed. Any nonconforming work shall be brought to the immediate attention of the contractor for resolution. A weekly report shall be submitted to the City; detailing the inspections and testing performed, listing any nonconforming work and resolution of nonconforming items. A final report shall be submitted to the building department prior to the Certificate of Occupancy being issued. Any unresolved discrepancies must be detailed in the final report. Contractor - The contractor shall provide the special inspector or agency adequate notification of work requiring inspection. The City approved plan and specifications must be made available, at the jobsite for the use of the special inspector and the City inspector. The contractor shall maintain all daily inspection reports, on site, for review by all parties. The special inspection functions are considered to be in addition to the normal inspections performed by the City and the contractor is responsible for contacting the City to schedule regular inspections. No concrete shall be poured or other work covered until approval is given by the City inspector. Buildigg Re2NJment: The building department shall review any revisions and addenda. The City inspector will monitor the special inspection functions for compliance with the agreement and the approved plans. The City inspector shall be responsible for approving various stages of construction to be covered and for work to proceed. Desien Professionals: The architect and/or engineer will clearly indicate on the plans and specifications the specific types of special inspection required and shall include a schedule for inspection and testing. The architect and/or engineer will coordinate their revision and addenda process in such a way as to ensure that all required City approvals are obtained, prior to work shown on the revisions being performed in the field. Owner: The project owner, or the architect or engineer acting as the owners agent, shall employ the special inspector or agency. ENFORCEMENT A failure of the special inspector or firm to perform in keeping with the requirements of the IBC, the approved plans and this document, may void this agreement and the Building Official's approval of the special inspector. In such a case a new special inspector and/or firm would need to be proposed for approval. A failure of the design and/or construction parties to perform in accordance with this agreement may result in a STOP WORK notice being posted on the project until nonconforming items have been resolved.
