Subject: Post Installed Anchors Structural Observation
Site Name: Bolin Residence

Sidesway Engineering has completed our structural observation of the post -installed anchors installed at
the aforementioned residence as part of shear wall upgrades required in the Sidesway calculation package
dated 11/07/18. The southeast wall required (3) 5/8"o anchor bolts with a 7" embed and (2) 5/8"0
holdown anchors with a 12" embed into the existing concrete basement wall.
We performed visual observation of the anchor installation in accordance with Section 1705.3 of the 2015
International Building Code. All threaded rod diameters were measured to be 5/8" and all hole diameters
were measured to be 3/4". The depth of the anchor bolt holes was measured at 10" and the depth of the
holdown anchor holes was measured at 14". All holes appeared to be clean of debris at the time of
installation. Simpson Set-XP Epoxy Adhesive with an expiration date of 08/19/20 was used. The epoxy
and anchors were installed accordance with the manufacturer's instructions.
Our observation was performed on May 8th subject to the standard of care applicable to professional
services and doesn't warrant or guarantee that all conditions were discovered. Please call if you have any
