Project Description

Multiple Maintenance locations at both existing aerial and underground. We will be splicing at the poles and our
existing ped's dog houses and vaults. Work zone has a North boundary of 158TH PL SW, South boundary of 68TH
Ave W and N Meadowdale Rd. We have 1 work zone in this boundary with only one zone being worked at one time.
There will be approximately 20 vehicles at a time with 4hr intervals, this includes traffic control set up and take down
with multiple locations. All roads have been highlighted to show all maintenance streets. CableCom will follow
standard traffic control for all roads. We have included overview map of full scope and maps broken out by work
zones..

SECTION C
Traffic Control and Public Safety. Traffic control and public safety shall be in accordance with City regulations as
required by the City Engineer. Every flagger must be trained as required by WAC 296-155-305 and must have

Restoration. Restoration shall be in accordance with City codes and Standards. All street cut trench work shall be

Insurance. The City may require the owner or contractor to provide proof of insurance acceptable to the City prior to
any permit approval.
Indemnification. The Applicant agrees to defend, indemnify, and hold harmless the City of Edmonds, its officials,
officers, employees, and agents against any injuries, damages, or claims of any kind or description whatsoever,
foreseen or unforeseen, including defense costs and attorney fees, resulting directly or indirectly from any act or
omission of the Applicant, its subcontractors, anyone directly or indirectly employed by them, and anyone for whose
acts or omissions they may be liable, arising out of, related to, or by reason of the granting of this permit; and
specifically including but not limited to all loss by the failure of the Applicant to fully or adequately perform, in any
respect, all authorizations or obligations under the permit. These indemnification requirements shall survive the
expiration, revocation, or termination of any resulting permit approval.
Acceptance of terms, conditions, and requirements. Applicant accepts the terms, conditions, and
requirements set forth in this Application and agrees they apply to any resulting permit and shall comply with them to
the satisfaction of the City Engineer. Applicant further agrees to comply with the conditions listed on the Submittal
Requirements; with all applicable city ordinances, including but not limited to ECDC Title 18; and with all applicable
requirements of state and federal law.
Statement of Applicant. I declare under penalty of perjury under the laws of the State of Washington that: I am a
licensed and bonded contractor; the information provided herein is correct and complete; and I am authorized by
owner to act on its behalf and have authority to bind the owner to this application.

Thanks for providing this additional clarification! I found an email from Natalie to Sunshine on June 18t" asking for
clarification on the second application, so I don't see that the second application was ever taken in. In order to keep
things simpler, I'll go ahead and change the scope of work under application ENG2021-0110 to be for the maintenance
splicing instead of the ped replacements. That way we don't have to withdraw ENG2021-0110 and charge for review
time for the withdrawn application.
In order for us to move forward with review of the maintenance splicing under application number ENG2021-0110,
please submit a new job packet with the following:
New ROW permit application form (our application form was updated since the previous one you filled out).
Please add a label to the map to indicate the work area within Edmonds' city limits.
All plan sheets and TCPs applicable to the work being conducted within Edmonds. Please don't include any TCPs
that are only applicable to the work being conducted within Lynnwood or Snohomish County.
We will be able to route the maintenance splicing for review once we receive this information. Feel free to contact me
with any questions.

Sorry for the confusions we have created here, I will try to clarify.
Originally we had submitted one application to replace the peds and do the maintenance splicing. We were
asked to break them apart into two applications which CableCom did. We have in our records that
oil 0 is the application number for the ped replacements which we do not need so we would like to cancel
that application. We also submitted the attached second application for the maintenance splicing that we still
do need, but don't show an application number for this submittal.
Clarification of this job -
We have a project we are working in Snohomish County primarily in the Lynnwood, Everett & Snohomish
County footprint. We do have some small sections that cross over to other City limits which is what this
application is for.
The project is to upgrade some of our system to take care capacity issues. With this we are installing fiber,
coax as needed and swapping out all devices located on the poles and in the pedestals (ped upgrades as
needed for new equipment). Once all new fiber/coax is placed we begin the splicing portion of the project.
When we do the splicing we need to do the entire limits of the zone at one time. Our goal is to be completed
with the splicing activity in the work zone within 4 hours.
We have received all the other required permits for this job (Snohomish County PUD & City of Lynnwood) and
hoping to resolve this as soon as possible. I would be happy to discuss this via phone or in a meeting if need
be.

I think there is some confusion on what we were wanting to do with our original permit. We need to access
these 2 pedestals' for maintenance splicing activities only. It appears from the picture below that the pedestal
may have been existing prior to the catch basin being installed. Does that change the requirements to
relocate the ped. I am available to discuss if that would be easier.

I am confused on this submittal. I believe this is the one we spoke about previously, is that correct?
We currently have the permit ENG2021-0110 in under the same job number. I have attached the original application
and the first resubmittal.
These are the comments on the resubmittal that I sent to Comcast on 5.5.2021:
1 have reviewed the resubmittal for this application and 1 have the following comments:
1. The application states that the dog house and pedestal are requested to be upgraded/replaced. What are the
specs for the replacement dog house and pedestal? Are the new ones proposed to be set in the same place as
the existing?
2. There is a City storm main at the eastern edge of 68th Ave W. How far will the new dog house and pedestal
be from the storm main and catch basins? The City requires 5' of horizontal separation. Please verify this is
possible —a field measurement from the catch basins would suffice.
3. Please also show the water, sewer, and storm mains on the plans. This can be a markup of the City's GIS Map
(link).
If this is a separate project from this permit, we will need more information so I know what exactly is proposed.
