2018-1-24 Field Report - L1 PT SlabD C I En s i n E E R S DATE: PROJECT: LOCATION: GENERAL CONTRACTOR: PRESENT: January 24, 2018 JOB #: Edmonds Mixed Use Building WEATHER: 201 Main Street, Edmonds, WA 98020 Edmonds 2020 LLC field report 14081-0137.00 Cold, Overcast, Some Rain Danielle Jacobs and Kyle Montzheimer (DCI), Ivan Nelson (Edmonds 2020 LLC), Tuven Tran (Krazan) DCI was on site Wednesday, January 24, 2018 to provide structural observation for the work in progress of the Level 1 P-T Slab. DCI walked the site with Ivan Nelson of Edmonds 2020 LLC, and Tuyen Tran of Krazan. The slab is scheduled to be poured Monday, January 29, 2018. The list below shall be completed and confirmed by the special inspector prior to the concrete pour. 1. The distributed tendons between Grids A & F near Grid 6.2 were chaired incorrectly between the highpoint at Grid 6 and mid -point at Grid 6.5. Verify that the chairs are rotated to provide a gradual parabolic slope to low point at Grid 7. 2. The concrete columns at Grids E/2 and B/4 had greater than '/2" overpour. Chip down concrete to achieve max'/2" overpour per 12/S3.01. 3. In multiple locations tendons were swept at or greater than 6:1 and hairpins were not yet installed. This was specifically observed at the banded tendon runs located at Grids B- C.5/5.5 & 6, E.5/1.5, and E.5/2. At these location, ensure tendons with sweeps greater than 6:1 have hairpins per 3/S5.01. 4. Near 1-2, there were sleeves within the tendon anchorage zone. Provide steel sleeves per 7/S5.02. 5. Near 1/1.8 and H.2/1.8, provide trim reinforcing around the groups of sleeves per 8/S5.02. At Grid G/2, the distributed tendons were too high over the column and did not have (2) minimum tendons running directly over the column per 12/S5.01. Shift minimum of (2) tendons directly over the column and route them below the banded tendons here to maintain 1"minimum concrete cover above. 7. At Grids D/2 and F/2, there were sleeves located between the stud rails at the columns. Please relocate the sleeve so (2) tendons can run directly over the column per 12/S5.01. 8. At C/1, the stud rails on the north side of the column were falling over. Straighten and secure to form prior to pour. 9. Near Grid C.8/4, there was an extra banded tendon head installed on the form. Remove prior to pour. field report Page 3 10. Near Grid C.5/4 there was a MEP sleeve adjacent to sweeping banded tendons. Either relocate the sleeve or provide a steel sleeve, hairpins around the tendons, and minimum 2" clear between the sleeve and tendons. 11. At the stair landing, provide #5T&B @ 12" oc. Provide dowels with standard hooks at the slab edge. Provide #5 T&B slab edge reinforcing per 10/S5.02. 12. At the northwest corner of the elevator, the tendons were sweeping too close to the slab edge/opening. Shift these over to maintain 3" clear minimum per 12/S5.02 and provide hairpins at the corner. Miscellaneous: 13. Verify slab is clear of debris prior to pour this was not complete at time of walk. 14. Ensure all electrical and plumbing ducts follow detail 4/S5.02. 15. At columns which do not align (column below slab is rotated in comparison to the column above the slab), #5 dowels should be used to spice steel which does not align in the column above. These observations should not be construed as a quality control inspection, nor shall anything herein be deemed to relieve the contactor from its obligations under the contract documents and all applicable codes. Observed deficiencies or variations from the approved contract documents have been noted above. The contractor and/or special inspector shall provide documentation and written verification of acceptable corrective action(s).
