DCI was on site Friday, March 30, 2018 to provide structural observation for the work in
progress of the Level 2 P-T Slab. DCI walked the site with Ivan Nelson of Edmonds 2020 LLC,
and Jerry of Krazan. The slab is scheduled to be poured Thursday, April 12, 2018. The list
below shall be completed and confirmed by the special inspector prior to the concrete pour.
1. Add steel sleeves around MEP penetrations occurring within a group of sweeping tendons.
This was observed at Grid 4.2 and Grid C.8.
2. Add hairpins at sweeping tendons per detail 3/S5.01. This was observed at Grids C.2 and
2.5, Grids C.2 and 5.5, Grids C.2 and 6, Grids D.2 and 6.8, Grids F.8 and 6.8, Grids H.2
and 2.2.
3. Verify all conduit is properly chaired and secured prior to pour. Verify 1" min clear between
conduit and all P-T tendons.
4. The rebar between Grids 5-7 and I -A' was not installed at time of P-T walk. Verify this is
installed prior to pour.
5. Column dowels were being installed during P-T walk. Verify installed prior to pour.

6. Add hairpins at tendon anchorage for group of 6 tendon at Grid G and 1.5.

7. Add hairpins at tendon anchorage for distributed tendons adjacent to banded at Grid A.5
and 1.5.

Add diagonal reinforcement bars at reentrant corners per detail 10/S5.02.
Between Grid lines 3-4 and A-B bottom mat reinforcement was not installed yet during the
P-T site walk. Verify installed prior to pour.

10. At Grids B and 2.5, there was one sleeve located between the stud rails at the column,
forcing the stud rail to skew. Relocate the sleeve.
11. Near Grid C.8 and 4.2 there were several MEP sleeves adjacent to sweeping banded
tendons. Either relocate the sleeves or provide steel sleeves, hairpins around the tendons,
and minimum 2" clear between the sleeve and tendons.
12. At the stair landing, provide #5T&B @ 12" oc. Provide dowels with standard hooks at the
slab edge. Provide #5 T&B slab edge reinforcing per 10/S5.02.
13. At Grids G and 4.5, and Grid G.5 and 4.5, more than one sleeve was provided in the stud
rail zone (as allowed by 11/S5.03). DCI will review.

Miscellaneous:
14. Verify slab is clear of debris prior to pour this was not complete at time of walk.
15. Ensure all electrical and plumbing ducts follow detail 4/S5.02.
This report is based on limited visual observations of structural components for general
conformance to the approved construction documents prepared by DCL There is no claim,
either stated or implied that all conditions were observed or analytically evaluated. These
observations should not be construed as a quality control inspection, nor shall anything herein
be deemed to relieve the contactor from its obligations under the contract documents and all
applicable codes. Observed deficiencies or variations from the approved contract documents
have been noted above. The contractor and/or special inspector shall provide documentation
and written verification of acceptable corrective action(s).
