DCI was on site Wednesday, May 23, 2018 to provide structural observation for the work in
progress of the Level 3 P-T Slab. DCI walked the site with Ivan Nelson of Edmonds 2020 LLC,
and Taylor of Krazan. The slab is scheduled to be poured Friday, May 25, 2018. The list below
shall be completed and confirmed by the special inspector prior to the concrete pour.
1. Install #4 nosing bar top at perimeter where wood shear wall is above.
2. Install hairpins per 3/S5.01 at tendon sweeps around plumbing cans as noted on site.
3. Provide hairpins at all sweeps greater than 1:6 per 3/S5.01 and where noted on site.
4. Provide trim reinforcement per 11/S5.02 for all groups of (5) or more sleeves.
Miscellaneous:
5. Verify slab is clear of debris prior to pour this was not complete at time of walk.
6. Ensure all stud rails are tied and secured prior to pour.
7. Ensure all electrical and plumbing ducts follow detail 4/S5.02.
This report is based on limited visual observations of structural components for general
conformance to the approved construction documents prepared by DCI. There is no claim,
either stated or implied that all conditions were observed or analytically evaluated. These
observations should not be construed as a quality control inspection, nor shall anything herein
be deemed to relieve the contactor from its obligations under the contract documents and all
applicable codes. Observed deficiencies or variations from the approved contract documents
have been noted above. The contractor and/or special inspector shall provide documentation
and written verification of acceptable corrective action(s).
