Reviewed detail 2 on S5.2 for the soldier pile splice. The web is a bolted connection with a splice plate and the
outside pile flanges are shown to welded with a 5/16" bevel weld. Inspected the fit -up prior to welding and
advised the contractor that the bevel size needed to be increased to provide the 5/16" fillet. This bevel was
ground with a grinder as required. Inspected the weld after completion and cooling and found the welding was
visually acceptable per AWS D1.1 visual inspection criteria. This was for the southern -most "H" pile.
To the best of our knowledge, the items inspected today are in conformance with approved plans and
specifications.

Observed the anchorage into hardened concrete of (16) 1/2"-diameter threaded rods horizontally, with a 2-3/4"
embedment, and (16) 3/4"-diameter threaded rods vertically, with a 4-1/2" embedment. Project -approved Hilti
HIT-HY 200-R (lot 1800 27 L2, expiration 02/2021) was used for attaching the FCD vault to the slab/foundation
per the approved contract documents, detail 6/CB-3.3 and per manufacturer specifications.
To the best of our knowledge, the items inspected today are in conformance with approved plans and
specifications.

Attended the City of Edmonds pre -construction meeting for the Waterfront Redevelopment project. Mayes
Testing Engineers' responsibility will be the following items listed, and discussed at this meeting:
1) Reinforced concrete including anchor bolts. There are revisions made to steel reinforcement at stairs and
cantilever wall.
2) Structural steel/welding (field and shop). WG Clark to coordinate shop welding inspection with their shop
fabricator.
3) Asphalt testing optional as discussed with City of Edmonds- Henry Schroeder.
The geotechnical engineer will do the site soils compaction testing.
WG Clark- Greg Jackson is the lead superintendent for both the Senior Center and Waterfront Redevelopment
projects.

On site to perform field density testing on waterline trench (12'x15'x4' deep) imported backfill subgrade. May
Valley Excavation performed the earthwork.
Soil Fill Placement Observation
At the time of arrival, noticed the material to be used as fill at the area were import materials visually classified as
5/8" crushed rock. Material was observed to be adequately moisture -conditioned and placed in lifts approximately
1' thick. Compaction efforts were applied utilizing a HoPac. The fill placed was observed to be firm and stable
after the compaction efforts. Field density tests were conducted utilizing a nuclear density gauge. The test results
met the minimum specified compaction requirement of 95%. Refer to the attached Field Density Test Report for
individual test data.
To the best of our knowledge, the items inspected today are in conformance with approved plans and
specifications.

Verified foundation wall footing reinforcing steel for bar size and grade, lap splice and development lengths and
cover and clearances. The installations are in conformance with the structural general notes and detail 7/S5.01.
Observed satisfactory pump placement and mechanical consolidation of CalPortland concrete mix 633331 (5000
f'c). The field test results were: Slump, 4-1/4"; Concrete Temperature, 70°F; and Entrained Air, 3.8%, which was
low. The application is not weather exposed, but the contractor and the onsite CalPortland Quality Control
representative were notified and made aware that the target per contract documents is 5.5%. One (1) set of (5)
4x8" cylinders was cast for compressive strength testing, as follows: (1) 3-day field -cure break; and (1) 7-day and
(3) 28-day lab -cure breaks (5000 f'c).

specifications.

The contractor placed approximately (10) cubic yards of CalPortland mix 633331 in forms for an 8"- thick retaining
wall on the west property line. Reinforcing steel in this form had previously been inspected and found to be in
conformance with approved plans and specifications. A representative sample of this mix was obtained, and (4)
4x8" cylinders were cast for laboratory curing and compressive strength testing.
Approximately (7.5) yards of this same mix were placed by the same contractor in a footing at the Edmonds
Senior Center in the adjacent lot.
To the best of our knowledge, the items inspected today are in conformance with approved plans and
specifications.

Observed the placement of (50) cubic yards of CalPortland mix 633331 concrete into the reinforced sheet pile
wall and pile cap south of the ramp/north stairs to the south end existing reinforced wall, near the north end of the
building. The reinforcing steel was inspected prior to placement for proper spacing, grade, size, lap splice
lengths, clearances and cleanliness. All reinforcement appeared to conform to the provided specifications, plans,
RFI's and details B, C and D/S3.2 and A/S4.1. At the transition between pile cap sections (details B/S3.2 and
D/S3.2), the top horizontal #5 bars need to be installed through the construction joint to provide the lap splice
development into the next phase of the pour. This was completed before concrete placement.
Concrete was properly placed using a pump truck, then mechanically consolidated using a vibrator. During
testing, the slump was observed at 8" at the truck chute, which is high (6" specified); but added water was in
tolerance, and the City Inspector and concrete foreman were notified. The temperature of the concrete was 760,
and air was measured at 5.1 %. One (1) set of (5) 4x8" cylinders was cast for compressive strength testing (f'c
5000 psi at 28 days).

specifications.

Observed and performed necessary in situ density tests for the backfill of the radiused retaining wall.
Upon arrival at the site, it was noted that the material to be used as backfill for the radiused retaining wall was
visually classified as imported gray sand with gravel (CalPortland Type 17). (See Mayes Testing Engineers
(MTE) Laboratory Compaction Characteristics of Soil Report 0016A, service date 06/01/20). The fill material was
observed to be adequately moisture conditioned and was placed in loose lifts approximately 12" to 18" thick.
Compaction efforts were applied utilizing a Komatsu PC138US excavator with a vibratory hoe -pack attachment.
At the time MTE departed, the contractor was planning on placing a second lift on the south end when more fill
material was received. The first lift was tested and passed at 2' below grade. All lifts observed today appeared
firm and stable during compaction efforts.
Field density tests were performed utilizing nuclear method ASTM D6938. The test results met the minimum
specified compaction requirement of 95% and 90% within 3' of the retaining wall, per recommendations by
geotechnical engineer Sean Gertz (Landeau Associates) provided via email. Refer to the attached Soil Field
Density Test Report for individual test data.
To the best of our knowledge, the items inspected today are in conformance with approved plans and
specifications.

Observed the placement of (14.5) cubic yards of CalPortland mix 633331 concrete into the reinforced sheet pile
wall (north end), the stair footing and the sheet pile cap (south end pour, Phase 2, below the precast cap). The
reinforcing steel was inspected prior to placement for proper spacing, grade, size, lap splice lengths, clearances
and cleanliness. All reinforcement appeared to conform to the provided specifications; plans; RFI's 31, 34 and
35; details B/S3.2 and A/S-2.4; and details on SK-05.
The concrete was properly placed using the truck chute, then mechanically consolidated using a vibrator. During
testing, the slump was observed at 6" at the truck chute. The concrete temperature was 75°F, and the air
entrainment was measured at 4.8%. One (1) set of (5) 4x8" cylinders was cast for compressive strength testing
(f'c 5000 psi at 28 days).
To the best of our knowledge, the items inspected today are in conformance with approved plans and
specifications.

Inspected the concrete placement of the promenade 6" structural slab -on -grade. The reinforcing steel conforms
to plan page S2.2 and associated details. Monitored and sampled the placement of (18) cubic yards of
CalPortland mix 63333i by truck chute, with mechanical consolidation. Air entrainment of the first load on arrival
was 4.2%. Cast (1) set of (4) 4x8" cylinders for compressive strength testing. Per S1.1, f'c of 5000 psi at 28 days
is required.
To the best of our knowledge, the items inspected today are in conformance with approved plans and
specifications.

On site to perform field density testing on frontage sewage line using imported backfill up to subgrade, close to
Railroad Avenue. WG Clark Construction Company performed the earthwork activities.
Soil Fill Placement Observation
At the time of arrival, noticed the material to be used as fill at the area was import material visually classified as
recycled concrete. Material was observed to be adequately moisture -conditioned and placed in lifts
approximately 1' thick. Compacting efforts were applied utilizing a HoPac and hand plate compactor. The fill
placed was observed to be firm and stable after the compaction efforts. Field density tests were conducted
utilizing a nuclear density gauge. The test results met the minimum specified compaction requirement of 95%.

Inspected (9) holes drilled into hardened concrete for the epoxy anchoring of wall dowels for the upper wall at the
north staircase (#5 bars at 12" on center each way) #5 bars x 43" long were anchored using Hilti HY20OR epoxy
(lot # 1759 27 L2, expires 02/2021). Holes were cleaned and ready for epoxy. Inspected the reinforcing steel for
north stair footings. Reviewed several drawings, RFI's and SSK's for the changes to the configuration. Found
the reinforcing to be per plans. Observed the placement of (19.5) cubic yards of CalPortland mix 633331 via
pump and vibrated for consolidation. Cast (1) set of (4) 4x8" cylinders for compressive strength testing (5000 psi
at 28 days).
To the best of our knowledge, the items inspected today are in conformance with approved plans and
specifications.

• The north beach stair risers and landings per detail A/S4.4.
• The bioretention planter walls per detail 3/CS4.1.

Performed concrete placement observation at the south stair upper footing. Prior to the placement, the
reinforcing steel installation was inspected per RFI 52, RFI 52.1, S1.2, detail 7/S5.1 and associated documents.
The installation was found to be substantially in conformance with the approved plan documents. Verified
clearances and cleanliness.
Monitored and sampled the placement of approximately (7) cubic yards of CalPortland concrete mix 633331 via
chute/direct discharge, with mechanical consolidation. Cast (1) set of (4) 4x8" cylinders for compressive strength
testing. Per schedule S1.1, a minimum f'c of 5000 psi at 28 days is required. The lab -cure sample cylinders
were placed in a box covered with plastic for initial site curing (see Concrete Sample Log below for more
information).

Inspected the reinforcing steel placed into the South stair walls. Found according to detail 7 on S5.1. Sampled
mix # 633331 from Cal Portland. Entrained air tested by QC and the result was 4.5%, the slump was 4.75",
concrete temperature was 81 OF and the ambient temp was 65°F. Cast (4) test cylinders for compressive strength
testing (5,000 psi required). All concrete placed by pump and vibrated for consolidation.
To the best of our knowledge, the items inspected today are in conformance with approved plans and
specifications.

Inspected the holes drilled for the epoxy anchoring of #5 reinforcing steel bars into the south stair walls for the
steps. Per the General Notes on S1.2, #5 bars are to be epoxy anchored with 7" of embedment. Upon
inspection, it was noted that the holes were 6" deep. Asked Dylan with WG Clark Construction about this; and he
stated that he directed the installer to drill 6" so that the back of the wall would not blow out during drilling, which
does occur when the holes are drilled too close to the outside face of the wall. Noted that the hook bars
anchored at the bottom of the stairs were 7-1/2" deep. All holes were brushed and blown clean, as required. Hilti
HIT-HY 200 (lot 1452 24 L2, expiration 06/2021) was used for the installation. The holes were filled, and the
reinforcing steel bars were installed.
PRELIMINARY INSPECTION
The engineer should review the reduced embedment for acceptance prior to concrete placement. Dylan has
made contact with the engineer and is waiting for a response.

be per plans.

anchored at the bottom of the stairs were 7-1/2" deep. All holes were brushed and blown clean, as required. Hilti
HIT-HY 200 (lot 1452 24 L2, expiration 06/2021) was used for the installation. The holes were filled, and the
reinforcing steel bars were installed.
PRELIMINARY INSPECTION
The engineer should review the reduced embedment for acceptance prior to concrete placement. Dylan has
made contact with the engineer and is waiting for a response.

Observed installation of pre -cast bulwark using FiveStar DP Epoxy Grout for the seawall/bulkhead. The FiveStar
DP Epoxy Grout was approved by email on 10/06/20 from project engineer, Blaine McRae, prior to anchor
installation, and was prepared according to manufacturer's specifications. The resin lot number was
5249651970, the hardener lot number was 5251702020 and the aggregate lot number was 21 FG-FIV-05008.
The (18) 3" diameter holes were drilled according to plan details (S3.2) with a depth of 12" +/- 3", meeting
minimum embedment depth of 8", according to plan details (S3.2). The holes were also drilled 12" +/- 3" on
center apart according to plan details (S3.2). The holes were roughened, cleaned and dried according to
manufacturer and project specifications, as well as possible considering environmental conditions, prior to anchor
installation. Epoxy -coated #6 bars were installed into bulwark prior to inspection. The 3" diameter holes were
then filled with FiveStar DP Epoxy Grout immediately prior to bulwark installation. The bulwark was then lifted
and placed by forklift with each of the epoxy -coated #6 bars inserted into each 3" diameter hole, meeting the
minimum 8" embedment depth (S3.2).
To the best of our knowledge, the items inspected today are in conformance with approved plans and
specifications.

Performed a reinforcing steel inspection for the radius weeding lawn concrete retaining wall (per L5.4) and for the
bulkhead structural pourback (per detail B/S3.2). Verified that the reinforcing steel was placed per the approved
contract document details and specifications.
Observed the placement of (12) cubic yards of CalPortland concrete mix 633331 (5000 psi at 28 days required)
for the items noted above. The concrete was placed by concrete pump truck and mechanically vibrated for
consolidation. A sample was taken, with (1) set of (4) 4x8" cylinders cast for compressive strength testing. The
field test results were:

Picked up nuclear gauge #24 from Mayes Testing Engineers Lynnwood, WA lab -cure to perform asphalt
compaction testing.
Upon arrival at site, spoke with WG Clark, Dylan, about re -compacting the base course subgrade disturbed by
equipment traffic prior to today, and observed some finishing of a little base course material grading including re -
compaction at the Waterfront Redevelopment side of the new asphalt paving lots.
Mayes Testing Engineers inspector, Keith Bellows, sampled 1/2" minus HMA material from the Cadman -
Woodinville plant for extraction and gradation. Was informed by Keith, this morning, of a 157.4 pcf for the Rice
value unit weight.
Ran in -place asphalt field density compaction tests today at the paved parking lots at the Edmonds Senior Center
and adjoining the City of Edmonds Waterfront Redevelopment projects. Nuclear density tests indicate compaction
effort by a 1.5-ton and a 5-ton roller meet the 91 % minimum compaction requirement. Asphalt temperature range
as delivered was between 265°F and 338°F. See attached Asphalt Field Density Test Report for additional
information.
Returned the nuclear gauge to the Lynnwood, WA lab at end of day per WSDOT requirement.
