CORRECTIVE ACTION TAKEN
On site for a follow up inspection to field report M7211251.0001, dated 05/26/2021, stating missing anchor not
installed due to reinforcing steel interference in the cold form steel wall at Passenger Room 102. Received the
attached documentation from Gensler -Don Henning with ASI Structures -Don Fuller approved fix to add a new
A3C wall aligned with the edge of the door frame and insert a new Simpson Strong bolt 2 (1/2x 3-3/4" ) at the
jamb for anchoring the cold form steel base track. Observed satisfactory installations and bolt torque at 60 foot-
pounds with effective embedment at 2-3/4". Installation of this anchor clears Mayes Terracon field report
M7211251.0001, dated 05/26/2021.
To the best of our knowledge, the items inspected today are in conformance with approved plans and
specifications.
