Traffic Control and Public Safety. Traffic control and public safety shall be in accordance with City regulations as
required by the City Engineer. Every flagger must be trained as required by WAC 296-155-305 and must have
certification verifying completion of the required training in their possession.

Insurance. The City may require the owner or contractor to provide proof of insurance acceptable to the City prior to
any permit approval.
Indemnification. The Applicant agrees to defend, indemnify, and hold harmless the City of Edmonds, its officials,
officers, employees, and agents against any injuries, damages, or claims of any kind or description whatsoever,
foreseen or unforeseen, including defense costs and attorney fees, resulting directly or indirectly from any act or
omission of the Applicant, its subcontractors, anyone directly or indirectly employed by them, and anyone for whose
acts or omissions they may be liable, arising out of, related to, or by reason of the granting of this permit; and
specifically including but not limited to all loss by the failure of the Applicant to fully or adequately perform, in any
respect, all authorizations or obligations under the permit. These indemnification requirements shall survive the
expiration, revocation, or termination of any resulting permit approval.
Acceptance of terms, conditions, and requirements. Applicant accepts the terms, conditions, and
requirements set forth in this Application and agrees they apply to any resulting permit and shall comply with them to

HWA GeoSciences Inc. (HWA) is planning to conduct a multi -phased exploration program in
support of the Phase 14 Waterline Replacement Project in Edmonds, Washington. Phase 1 of
the exploration plan will consist of drilling three (3) geotechnical borings to determine soil and
groundwater conditions. Phase 2 of the exploration plan will consist of conducting five (5)
pavement cores to determine asphalt thickness and subgrade condition. The approximate
locations of the proposed explorations are presented in the attached proposed Exploration Maps,
Figures I and 1B. All of our explorations will be conducted within the Right of Way (ROW)
of City of Edmonds.
SCOPE OF WORK:
Phase 1: Geotechnical Borings
As part of Phase 1, we propose to drill three (3) machine drilled borings designated BH-1
through BH-3 to assess the subsurface soil conditions along the project alignment. These boring
will be drilled to a depth of 15 feet. Borings will be conducted in the roadway pavement, within
the right of way of City of Edmonds.
Exploratory geotechnical borings will be advanced by Geologic Drilling, Inc. of Bellevue,
Washington using a limited access track mounted drill rig advancing a hollow -stem auger. The
boreholes will be 6- to 8-inches in diameter. Standard Penetration Test (SPT) sampling will be
performed in the boreholes at intervals of 2.5 to 5 feet using a 2-inch outside diameter split -
spoon sampler and a 140-pound drop hammer. Soil cuttings from the borings will be transported
off site for disposal.

Upon completion, each borehole will be abandoned per Washington State Department of
Ecology (DOE) regulations. The boreholes will be filled with bentonite chips to the bottom of
pavement base course section. Any unconsolidated soil directly below the pavement section will
be compacted using a hand operated slide compactor. Six inches of crushed gravel will be placed
and compacted in the borehole to the base of the existing pavement. Rapid setting non -shrink
grout such as Blue -Line Non -Shrink grout or equivalent, dyed to match existing pavement will
be used to patch the affected surface.
A geotechnical engineer or a geologist from HWA will log each exploration and record all
pertinent information.
Phase I Duration of Work: We anticipate that drilling each of the proposed machine drilled
geotechnical borings will take approximately 2 to 3 hours including setup and clean-up. All
phase 1 geotechnical borings will be performed during the daytime between the hours of 8:00
AM to 5:00 PM. Assuming the tentative work hours are approved, we expect machine drilled
borings will require one day of work. At this time, HWA has scheduled the drilling to be
conducted on June 22, 2023. If required permits are not obtained by June 13, 2022, HWA will
have to reschedule the drilling for a later time. In our experience, it would take about 3 to 6
weeks to reschedule drilling dates.
Phase 2: Pavement Coring
Pavement cores designated Core-1 through Core-5 will be conducted using a 6-inch-diameter
hand -portable core -drill rig. All cores will be collected from the center of the travel lanes.
Setup for the coring process consists of securing and leveling the portable drill -press to the
pavement surface, and subsequently attaching the drill motor and core barrel. Water is injected
into the core barrel during drilling to facilitate coring operations. Core cuttings are vacuumed
from the roadway as they are produced and disposed of off -site. After the core is removed from
each location, a geologist will use a hand auger to carefully dig through the subgrade soils to a
depth of 1 to 2 feet and log soil conditions. Samples may be collected to properly characterize
the subgrade soils.
The core holes will be backfilled with crushed rocks to the bottom of the asphalt layer and will
be patched with rapid setting non -shrink grout such as Blue -Line Non -Shrink grout or
equivalent, dyed to match existing pavement.
Phase 2 Duration of Work: The coring process will take approximately two hours at each
location. All phase 2 pavement cores will be performed during the daytime between the hours of
8:00 AM to 5:00 PM. Assuming the tentative work hours are approved, we expect pavement
coring will require two (2) days of work. At this time, HWA has scheduled the pavement coring
to be conducted by HWA in-house team on June 201h and 21', 2023. If required permits are not
obtained by June 13, 2022, HWA will have to reschedule the pavement coring for a later time.

UTILITY LOCATES
HWA will mark the locations of the proposed exploration and submitted utility locate requests to
the One -Call utility locate service. HWA will be working with the utility owners to determine the
location of the buried utilities in the vicinity of our proposed explorations. As a matter of
standard practice, HWA and drilling subcontractor will duplicate locate requests prior to drilling.
The proposed exploration locations are subject to minor location changes based on subsurface
utility locations.
TRAFFIC CONTROL PLANS
Conducting the pavement cores Core-1 through Core-5 and borings BH-1 through BH-3 will
require temporary single lane closure with alternating flagger controlled one-way traffic. Traffic
control will be conducted in accordance with WSDOT Standard Work Zone Traffic Control
Guidelines M54-44.09 (2021) plan TC-1 for One Lane, Two Way Traffic Control with Flaggers
(Appendix A, Figure A-1). Only one lane closure at a given time will be performed.
The implementation of all the traffic control measures will be in accordance with WSDOT
Standard Work Zone Traffic Control Guidelines M54-44.09 (2021) and FHWA MUTCAD
(2009).
