Project Description
Include an overall narrative description of the proposed work. Be specific and detailed:
Describe infrastructure to be impacted, such as, poles, wires, conduit, utility mains, etc.
Include whether this is construction, maintenance, or repair.
Include description of proposed construction methods, (e.g., bores, trenches, overhead wires proposed, depth for
underground utilities, etc.)
This project will entail: -Drilling and installing one flush -mount groundwater monitoring well within the property line along Dayton Street. -
Drilling two soil borings that will be backfilled immediately after collection of soil samples in the sidewalk along 5th Avenue South.The well and
soil borings will be drilled to an approximate depth of 15 feet. Closure of the sidewalks will be required during installation of the well and soil
borings drilling. Sidewalk closure is not expected to be more than 72 hours.The installation of the well will take approximately four hours. The
proposed activities are not expected to disrupt the asphalt, however, the sidewalk will be restored to City standards. The frequency of groundwater
sampling will depend on the results, but will likely be quarterly. The groundwater monitoring well is estimated to be active for approximately 5
years, but the actual duration will depend on the sample results and regulatory agency requirements.

Traffic Control and Public Safety. Traffic control and public safety shall be in accordance with City regulations as
required by the City Engineer. Every flagger must be trained as required by WAC 296-155-305 and must have
certification verifying completion of the required training in their possession.

Insurance. The City may require the owner or contractor to provide proof of insurance acceptable to the City prior to
any permit approval.
Indemnification. The Applicant agrees to defend, indemnify, and hold harmless the City of Edmonds, its officials,
officers, employees, and agents against any injuries, damages, or claims of any kind or description whatsoever,
foreseen or unforeseen, including defense costs and attorney fees, resulting directly or indirectly from any act or
omission of the Applicant, its subcontractors, anyone directly or indirectly employed by them, and anyone for whose
acts or omissions they may be liable, arising out of, related to, or by reason of the granting of this permit; and
specifically including but not limited to all loss by the failure of the Applicant to fully or adequately perform, in any
respect, all authorizations or obligations under the permit. These indemnification requirements shall survive the
expiration, revocation, or termination of any resulting permit approval.
Acceptance of terms, conditions, and requirements. Applicant accepts the terms, conditions, and
requirements set forth in this Application and agrees they apply to any resulting permit and shall comply with them to
the satisfaction of the City Engineer. Applicant further agrees to comply with the conditions listed on the Submittal
Requirements; with all applicable city ordinances, including but not limited to ECDC Title 18; and with all applicable
requirements of state and federal law.

owner to act on its behalf and have authority to bind the owner to this application.

GENERAL NOTES -TRAFFIC CONTROL
It is the responsibility of the contractor performing work to install and maintain the traffic control devices as shown herein, as well as any such additional
traffic control devices as may be required to ensure the safe movement of vehicular and pedestrian traffic through or around the closure area and provide
maximum protection and safety to workers.
All traffic control devices shall be kept in their proper position at all times and shall be repaired, replaced, or cleaned as necessary to preserve their
appearance, continuity and capability.
All traffic control devices shall conform to the latest accepted edition of the Manual on Uniform Traffic Control Devices ( M.U.T.C.D.).
Any revisions to these drawings shall be approved in writing by the Engineer of Record and the Agency having jurisdiction.
The Agency Traffic Engineering Division reserves the right to observe the traffic control operations and to make any necessary changes. Any directed
changes shall supersede these plans and shall be implemented at the sole expense of the contractor.
Contractor shall provide flagmen as necessary to give adequate warning to traffic or to the public of any dangerous conditions to be encountered.
Contractor shall remove temporary traffic delineation, signage, and other devices when no longer required, and shall restore areas to original conditions.
Contractor shall cover existing signs where they conflict with construction detours and signing.
All open excavation or construction work shall be a minimum of 5' from any operating traffic lanes.
Contractor shall post uneven pavement signs, and ramp vertical pavement offset of 1 inch or more with asphalt for smooth transition.
The contractor shall provide for access to all adjacent properties during work hours. Construction operations shall be conducted in such a manner as to
cause as little inconvenience as possible to abutting property owners/operators.
All signs shall be reflectorized and standard size.
The contractor shall provide for safe pedestrian access at all times.
