The proposed facility has been reviewed for consistency with the Federal Communications Commission (FCC) regulations
for radio -frequency radiation. Based on this review, the following determination has been made (check all boxes that
apply):
❑X The proposed personal wireless service facility, together with all other transmitting antennas on the site, is
consistent with all FCC radio frequency radiation exposure guidelines. Also, the proposed personal wireless
service facility does not exceed the threshold at which an Environmental Assessment is required according to
FCC Title 47 Code of Federal Regulations (CFR) Section 1.1307, Actions that may have a significant environmental
effect, for which Environmental Assessments (EA's) must be prepared.
(This proposal does not need to be reviewed by the Seattle -King County Department of Public Health (SKCDPH).
The SKCDPH certification letter is attached.)
❑ The proposed personal wireless service facility, together with all other transmitting antennas on the site, is
consistent with all provisions of FCC radio frequency radiation exposure guidelines. However, the proposed
personal wireless service facility exceeds the threshold at which an Environmental Assessment is required
according to FCC Title 47 CFR Section 1.1307, Action that may have a significant environmental effect for which
Environmental Assessments (EA's) must be prepared.
(The Environmental Assessment and NIER Report for the proposal are attached and have been reviewed by the
SKCDPH. The SKCDPH certification letter is attached).
Required Affidavit of Qualification and Certification
Measurements and calculations regarding compliance with FCC regulations shall be certified by the person responsible
for them, and an affidavit shall be attached to this statement that includes an explanation of the protocol, methods,
equipment, and assumptions used. The affidavit shall also include a statement that the person responsible for the
measurements and calculations is licensed Professional Engineer (PE) and provide the Engineer's state PE license
number (SMC 25.10.310.5.C).
