A Krazan & Associates, Inc. representative visited the site per the contractor's request to conduct a welding inspection for
the metal decking. We reviewed general note S.001 (Metal roof or floor decking) and detail 2/S.601 for the required
decking material and welds. We audited the welder's qualification (WABO card) and consumables. Metal deck type C
(non -composite), Product 20G 7/8" Formdek G90, was placed in the short -span direction and welded. We noted the
following discrepancies and addressed to the contractor before leaving the site.
1) The plate (1/"x4"x cont.) that called out per 2/S.601 was omitted per EOR, per the contractor's verbal info.
2) We noticed that the metal deck was supported by the existing channel about ''/z" — 5/8". A minimum of 2-inch support
is required per general note #5, under Metal Roof or Floor Decking section.
3) The size of the puddle welds was only — 1/" @ 12" OC, the required size per 2/S.601 is 5/8" puddle welds @ 12" OC.
4) No side -lap connections. Method and spacing of side -lap connections were not specified, refer to MFR's instructions.
To the best of my knowledge, the auove WAS NOT performed in accordance with the approved plans, specifications and regulatory requirements.
