A Salvage Assessment is required for projects that involve demolition with an area of work greater than 750
square feet or the construction value is greater than $75,000. The assessment should be submitted at permit
intake if possible but is required prior to issuance. Be sure that the correct person fills out this form:
• If a whole building is being deconstructed or demolished, the form must be completed by a salvage verifier that meets
one of the following:
■ An established salvage and reuse retail company
■ A licensed contractor specializing in deconstruction
A demolition company with knowledge of local and current salvage retail markets
• The salvage verifier may use an alternate form as long as specific materials have been identified for salvage.
• A list of possible salvage verifiers may be found through resources such as The Northwest Building
Salvage Network: httn://nbsnseattle.org/.
• The salvage verifier may check off the box below if there is nothing of value to salvage.
• The owner or owner's representative may fill out the DSA if the project involves interior alterations that include
demolition or if materials being removed from the project are going to be used in the new project or at an

Materials will be removed by Second Use on a cost neutral basis for tax deduction purposes and abiding by the following criteria:
1. Proposal does not include time and materials for covering exterior openings or protecting finishes.
2. All items to be retained by owner must be protected or moved from work area prior to Second Use arrival.
3. Proposal is for one continuous mobilization.
4. All waste generated from salvage activities to be left on -site in a predetermined location or dumpster.
5. All doors listed to include jamb and hardware.
6. Final list of actual items salvaged to be determined at time of extraction due to: material availability, job site conditions, and/or discovered
unsuitability of materials for reuse.
7. Any changes to job site conditions which become a safety risk or unnecessarily impedes the efficient removal of materials listed will make this bid
void.
'TBD, short for "To Be Determined" indicates an item that may not survive extraction process or may be dropped in priority due to un-forseens on
jobsite.
