(Amec Foster Wheeler), conducted a Limited "Good Faith" Asbestos and Lead (Pb) Survey at the Edmonds Medical Plaza building located at 21701 76th Ave W, Edmonds, Washington. This sampling was limited to Suites 201 and 202 of the structure. The sampling was completed on March 9, 2018, by Asbestos Hazard Emergency Response Act (AHERA)-certified Building Inspectors, Ms. Susan Kimball and Mr. Anthony Bird. The Limited "Good Faith" Asbestos and Lead Survey included the sampling of 52 suspect asbestos - containing materials (ACM) and 13 suspect lead -containing paint (LCP) coatings. This assessment also included taking an inventory of PCB -containing light ballasts, and mercury -containing fluorescent light tubes, switches, and thermostats. SCOPE OF WORK Amec Foster Wheeler provided an AHERA-certified Building Inspector to survey the renovation project areas in suites 201 and 202. The exterior of the building was not included in this survey. The survey was intended to identify and evaluate the presence and quantity of ACM associated with the planned renovation projects. Amec Foster Wheeler understands that in the project areas, the renovation will likely impact all interior materials within Suites 201 and 202. SITE DESCRIPTION The Edmonds Medical Plaza building is located at 21616 76m Avenue in Edmonds, Washington. The building is a multi -story medical building of wood framed construction. Floor plans of units sampled are included in Attachment 5. LIMITATIONS OF THE INSPECTION The conclusions of the report are professional opinions based solely upon visual site observations, and interpretations of analyses as described in this report. The limits of the surveyed work area were based on discussions with Ms. Jennifer Gloeckner of Healthcare Realty prior to the assessment and are limited to the interior components of suites 201 and 202. The opinions presented herein apply to the site conditions existing at the time of the investigation and interpretation of current regulations pertaining to ACM. Therefore, opinions and recommendations provided herein might not apply to future conditions at the site. The current regulations should always be verified prior to any work involving asbestos or other regulated materials. This survey is not intended to be used as an abatement design document. All existing conditions, quantities, and locations should be verified prior to abatement. The purpose of the survey is to reasonably test for evidence of asbestos in the limited areas described above for the purpose of future common area renovations. It should be noted that no assessment can be comprehensive or exhaustive enough to completely eliminate the possibility that regulated materials are present at the Edmonds Medical Plaza building. Therefore, the completion of this or any survey for hazardous materials should not be considered a warranty or guarantee that these materials do not exist, even if they are not detected through a survey. METHODOLOGY Asbestos -Containing Building Materials The limited "good faith" asbestos survey was performed by an AHERA-certified building inspectors, Mr. Anthony Bird and Susan Kimball, in accordance with a sampling protocol appropriate for the renovation of commercial building spaces. The sampling protocol was modeled after Environmental Protection Agency (EPA) regulation 40 Code of Federal Regulations (CFR) 763, Puget Sound Clean Air Agency (PSCAA) Regulation III, Article 4, and by Washington State Department of Labor and Industries Division of Occupational Safety & Health (DOSH) Regulation Washington Administrative Code (WAC) 296-62-077021. Suspect ACM was grouped into homogeneous sampling areas (HSA) and categorized according to 40 CFR 763, as thermal systems insulation (TSI), surfacing material, or miscellaneous material. Ms. Jennifer Gloeckner Healthcare Realty March 28, 2018 Page 3 of 9 I amec 44 foster wheeler The sampling plan included, at a minimum, the collection and analysis of samples as follows: • Thermal System Insulation - In a distributive manner, a minimum of three (3) samples of each HSA that was not assumed to contain asbestos. - At least one (1) bulk sample from each homogenous area of patched TSI if the patch was less than 6 square feet. • Surfacing Material - In a distributive manner, a minimum of three (3) samples collected from each homogenous area that was 1,000 square feet or less. - A minimum of five (5) samples collected from each homogenous area that was greater than 1,000 square feet but less than or equal to 5,000 square feet. - A minimum of seven (7) samples collected from each homogenous area that was greater than 5,000 square feet. • Miscellaneous Material - Ina distributive manner as deemed sufficient by the Inspector, at least one (1) sample was collected of each suspect miscellaneous material not assumed to contain asbestos. • Non -Suspect Materials - According to 40 CFR 763-86(4), sampling of the following materials are not required where the accredited inspector has deemed the material to be fiberglass, foam glass, rubber, or other recognized non -ACM. Samples were collected by carefully removing small portions of the suspect material with a sharp knife or other hand tool suitable for the material being sampled. Each sample was placed in a labeled plastic container immediately after collection. Sample containers were then placed in a large re - sealable plastic bag for transportation to the laboratory. The sampling instrument was wiped with a clean moist cloth to decontaminate the tool and minimize the potential release of asbestos fibers or contamination of subsequent samples. Data pertinent to each sample (e.g., date, sample number, material description, and material category) was recorded on a field data sheet. The material determination of friability was made by the inspector in the field. The table listing the results further clarified that when a layered sample contains at least one friable layer, then the entire composite sample is determined to be friable. Lead -Containing Paints and Materials Paint chip samples were obtained by carefully scraping the paint layers away from the substrate with a stainless -steel knife blade. Approximately 1 square inch of paint coating was removed for each sample. Samples were then placed into 2-ounce, puncture proof, polyethylene bags and labeled with individual sample numbers. Samples were assigned a unique identifier when placed in sampling containers. Universal Wastes Survey Verifying the presence or absence of universal wastes, either visually or by laboratory analysis, was beyond the scope of this survey. The survey did include quantifying potentially PCB -containing fluorescent light ballasts. All ballasts were assumed to contain PCBs. A similar assumption applies to mercury (Hg) potentially present within fluorescent lamps in fluorescent light fixtures, high intensity discharge lamps, compact fluorescent light bulbs, thermometers, and thermostats. A summary of inventoried universal waste is located in Table 3. Laboratory Analysis Asbestos bulk samples and chain -of -custody submittal sheets were delivered to NVL Laboratories, Inc. (NVL), in SeaTac, Washington, for asbestos analysis. NVL participates in the NVLAP (National Voluntary Laboratory Accreditation Program) for quality control procedures. As specified in 40 CFR Chapter 1 (1-1-87 edition) Part 763, Subpart F, Appendix A, each sample was analyzed using Polarized Light Microscopy (PLM) with dispersion staining techniques, in accordance with EPA Method 600/R-93/116. The detection limit for this type of analysis is approximately one percent (by volume). Materials containing more than 1 percent asbestos are considered to be an ACM. The laboratory analytical data report, chain -of -custody forms, laboratory certification, and inspector certification are provided in the attachments. Paint chips samples were analyzed for percent of lead by weight by NVL using flame atomic absorption according to EPA Method 7000B. NVL participates in the National Environmental Laboratory Accreditation Program (NELAP) and employs American Industrial Hygiene Association (AIHA) quality control procedures. Laboratory analytical data reports and chain -of -custody forms are provided in the attachments. RESULTS Asbestos Amec Foster Wheeler collected 52 samples of suspect ACM. Two of the materials sampled were identified as asbestos -containing materials. These materials are described below in Table 1. A summary of all bulk asbestos sampling and results is presented in Attachment 1, and includes the sample number, homogenous material description, material classification, and analytical results. The Washington State Department of Community, Trade, and Economic Development (CTED) defines lead -based paint as coatings with a concentration of lead greater than or equal to 0.5 percent by weight (5,000 fag/g, 5,000 mg/kg, or 5,000 parts per million [ppm] by weight). However, DOSH considers any detectable concentration of lead to be a potential hazard during construction activities. A summary of the lead containing paint chip samples identified is described in Table 2 below. If additional suspect building materials not identified specifically in this report as either ACM or non -ACM are identified during construction activities, they should be treated as asbestos containing until sampled by an AHERA Building Inspector and proven to not contain asbestos through laboratory analysis. The contractor should use caution when performing renovation or demolition activities within the project areas. Concealed materials may be encountered during the renovation project. ACM may be located between walls, above ceilings, in pipe chases, or other inaccessible areas. If additional suspect building materials not identified specifically in this report as either ACM or non -ACM are identified during construction activities, they should be treated as asbestos -containing until sampled by an AHERA Building Inspector and proven to not contain asbestos through laboratory analysis. Amec Foster Wheeler recommends that the contractor have a copy of this report during renovation activities at the Edmonds Medical Plaza building. Lead -Containing Paint Coatings DOSH considers any detectable concentration of lead to be a potential hazard during construction activities. Amec Foster Wheeler completed representative testing of painted coatings within the site building. Based on this testing, only paint coatings on windows in Suite 201, and on drywall in room 1 of Suite 202 are considered to be lead -containing paint. See Table 2 above for a complete inventory of lead -containing paint coatings. Renovation activities will disturb lead -containing paint coatings and potentially result in worker exposure to lead. Necessary precautions should be taken to prevent or minimize the release of lead in the form of dust, fumes, or mists into the air or into surrounding environments. All workers and supervisory personnel who will be at the job site must be informed of the existing lead hazards, be lead -awareness trained, and be knowledgeable on the precautions and housekeeping procedures necessary to minimize the lead exposure hazard. For work on building components containing lead, which may result in personnel exposures, the contractor must assess the hazard. Based on the assessment, and previous similar work and exposure monitoring results, the contractor may have to provide any or all of the following for employees per WAC 296-155-176: Ms. Jennifer Gloeckner I Healthcare Realty amec 44 March 28, 2018 foster Page 7 of 9 wheeler • Respiratory protection; • Protective clothing; • Clean change areas; • Clean hand -washing facilities; • Biological monitoring to consist of blood sampling and analysis for lead and zinc protoporphyrin levels; and • Hazard communication training. Initial employee exposure monitoring must be conducted for each separate task involving the handling of LCP. If 8-hour time -weighted average (TWA) exposures exceed the action level of 30 micrograms per cubic meter (pg/m3), the contractor must continue to conduct periodic air monitoring at specified intervals, and institute medical surveillance and comprehensive training programs. If the DOSH 8-hour TWA permissible exposure limit of 50 pg/m3 for lead is exceeded, more stringent and additional requirements become effective, such as engineering controls, respiratory protection, regulated work areas and warning signs in lead work areas. The general contractor performing the demolition work should be informed of the presence of lead in the project area. All personnel impacting who come into contact with LCP (or other lead -containing materials) should be provided additional training concerning the health effects of lead, proper work methods, appropriate use of personal protective equipment, and regulations governing lead exposures. Air monitoring to assess lead exposures should be performed for all personnel involved in the demolition process where LCP is removed. Universal Wastes The Washington State Dangerous Waste Regulations, WAC 173-303, designates that discarded transformers, capacitors, or bushings containing PCBs at concentrations of 2 ppm or greater be treated as a PCB -containing material. Light ballasts fall under this regulation. Previous regulations dictated that any material with less than 50 ppm PCBs could be labeled as a non -PCB containing material. Because of this regulatory change, Amec Foster Wheeler recommends that all light ballasts be tracked, removed, handled, and disposed of in an appropriate manner. The ballasts with the "non - PCB" label (or something similar) shall be packaged for recycle by an approved recycling facility. Many fluorescent light tubes, HID lamps, and thermostats contain mercury that is harmful to the environment and human health thus classifying the material as dangerous waste. The EPA and Ecology have placed these materials in a special category of dangerous wastes known as universal Ms. Jennifer Gloeckner Healthcare Realty March 28, 2018 Page 8of9 I amec 44 foster wheeler waste. Some of the requirements included within the Universal Waste Rule for Dangerous Waste Lamps (WAC 173-303-573) include: • Immediately place lamps showing evidence of leakage, damage, etc. in a container following removal. • Containerize in closed, structurally sound, compatible containers. • Cardboard containers may be used for inside storage only. • Labeling container required: "Waste Lamps," or "Universal Waste Lamps." • Track the length of time since waste lamp generation. Acceptable methods of proof include date on label, inventory system, etc. • Respond immediately to potential releases. If determined to be a release, contain and determine whether the release is designated as a dangerous waste. If so, manage the release as specified in WAC 173-303. • Disposal of universal waste as general or construction debris is not permitted. • The crushing of fluorescent light tubes on -site is not allowed. In addition, measures should be taken to prevent breakage of fluorescent light tubes while the light tubes are in transit to their destination. • Provide training to employees on the proper handling and emergency procedures of universal waste lamps. • Track shipments of universal waste lamps with records (invoice, manifest, etc.) kept for a minimum of 3 years. Examination of these samples was conducted for the presence of identifiable asbestos fibers using polarized light microscopy (PLM) with dispersion staining in accordance with both EPA 600/M4-82-020, Interim Method for the Determination of Asbestos in Bulk Insulation Samples and EPA 600/R-93/116 Method for the Determination of Asbestos in Bulk Building Materials. For samples containing more than one separable layer of materials, the report will include findings for each layer (labeled Layer 1 and Layer 2, etc. for each individual layer). The asbestos concentration in the sample is determined by calibrated visual estimation. For those samples with asbestos concentrations between 1 and 10 percent based on visual estimation, the EPA recommends a procedure known as point counting (NESHAPS, 40 CFR Part 61). Point counting is a statistically more accurate means of quantification for samples with low concentrations of asbestos. The detection limit for the calibrated visual estimation is <1 %, 400 point counts is 0.25% and 1000 point counts is 0.1% Samples are archived for two weeks following analysis. For samples containing more than one separable layer of materials, the report will include findings for each layer (labeled Layer 1 and Layer 2, etc. for each individual layer). The asbestos concentration in the sample is determined by calibrated visual estimation. For those samples with asbestos concentrations between 1 and 10 percent based on visual estimation, the EPA recommends a procedure known as point counting (NESHAPS, 40 CFR Part 61). Point counting is a statistically more accurate means of quantification for samples with low concentrations of asbestos. The detection limit for the calibrated visual estimation is <1%, 400 point counts is 0.25% and 1000 point counts is 0.1% Samples are archived for two weeks following analysis. Examination of these samples was conducted for the presence of identifiable asbestos fibers using polarized light microscopy (PLM) with dispersion staining in accordance with both EPA 600/M4-82-020, Interim Method for the Determination of Asbestos in Bulk Insulation Samples and EPA 600/R-93/116 Method for the Determination of Asbestos in Bulk Building Materials. For samples containing more than one separable layer of materials, the report will include findings for each layer (labeled Layer 1 and Layer 2, etc. for each individual layer). The asbestos concentration in the sample is determined by calibrated visual estimation. For those samples with asbestos concentrations between 1 and 10 percent based on visual estimation, the EPA recommends a procedure known as point counting (NESHAPS, 40 CFR Part 61). Point counting is a statistically more accurate means of quantification for samples with low concentrations of asbestos. The detection limit for the calibrated visual estimation is <1%, 400 point counts is 0.25% and 1000 point counts is 0.1 % Samples are archived for two weeks following analysis.
