12.1 Sunset Ave North, Permit #b1d20080463
Duane,
On March 31, 2009 Your contractor performed a water main tic in within Sunset Avenue
right-of-way. A new pipe section was installed with a series of toes and valves to
accommodate one fire hydrant, one I" copper domestic service, one 2" poly domestic
Service, one 6" ductile iron fire line and two in-line resilient gate valves on the City water
main. After installation of this pipe section was complete and the main water system in
the area was turned back on, the contractor installed the fire hydrant. This was the extent
of the work inspected by Jeanie McConnell and JoAnne Zulauf oil March 31"' and the
water line crossings for each service was to be installed the following day.
Oil April 1, 2009 the City noted an inspection was not called in for the install of the pipe
crossings. Engineering inspector Jennifer Collins contacted you to confirm the water
lines would be installed and it was noted the contractor would be ready for inspection by
I Pill.
Upon arrival to the site around 2pm, the entire trench had been backfilled. Inspectors
Jeanic McConnell and Jorinifor Collins were not able to inspect any of the pipe runs that
had been installed. The contractor informed LIS that CDF had been placed in the area of
the fire hydrant and valve cluster. City inspectors could verify use of 12" PVC around
the valves, as required, but Could not verify the extent of CD .
With regards to traffic control, Sunset Avenue was completely blocked by equipment and
vehicles and was not open to the general public. The approved traffic control plan for the
pro ' ject did not allow for closure of the entire roadway and the City was not informed of
the need for closure prior to the contractor closing the road.
The 6" fire line, and I" & 2" domestic services were stubbed into the property at the
same elevation, about mid -slope, and about 10' west of the property line. A meter setter
had been installed on the 1" copper service, but was placed so close to the ductile iron
pipe that a meter and meter box cannot be: installed,

The following inspections, corrections and plan revisions are required prior to acceptance
of the water system:
1) Placement of CDF —
i CDF shall be exposed near the curb line and at the western limits of the CDF
placement so the extents of CDF placed within the right-of-way can be
verified.
An inspection was called in for this today and Engineering Inspector
Jennifer Collins noted the CDF had not cured. A dry mix was used and the
contractor informed that due to the rainy weather it was taking longer to
harden. This section of right-of-way shall not be permanently restored
until the City can verify the CDF has cured and/or the material has been
removed and replaced with 3/4 sack mix CDF.
2) 1" and 2" domestic services—
The
ervices—
The 1" copper and 2" poly water service lines shall be stubbed out to the
property line, versus interior of the property, and shall be vertically re -aligned
to tie into the elevation in which the water meters will be set at the edge of the
right-of-way.
The water service lines shall be exposed within the right-of-way to the extent
necessary to provide a smooth transition from the depth in which the service
lines were placed in Sunset Ave to their final termination point near sidewalk
grade. The service lines shall not he bent in anyway, nor shall fittings be
placed to accommodate for the elevation change.
The 2" water service line shall be flushed clean prior installation of the setter
and meter.
If the 2" meter setter is set ahead of the scheduled meter installation then the
contractor shall obtain a jumper (spacer) from the City Public Works Dept.

"i A plan shall be submitted to the City indicating that the shift in horizontal
alignment to the north will not effect installation of the RPBA's in hotboxes
on private property, consistent with City standards.
3) 6" Fire Line -
The fire line shall remain inactive until a pressure test has been completed.
i:E The fire line shall be tested at 200psi for 2 hours.
D The pressure test shall not be performed prior to confirmation that the CDF
has set up.
It would make sense to test this line after the foundation has been backfilled
and the fire line vault has been set.
C] The fire line shall be flushed clean prior to installation of the double check
detector assembly.
4) Compaction Tests —
3:J Compaction test reports shall be submitted to the City for that portion of the
trench section backfilled with crushed rock. 95% compaction is required.
Compaction tests shall be taken again for that portion of the, trench that is dug
up in order to re -align the water services.
As we discussed, please SUbrnit a staging and haul route plan for the upcoming concrete
pours.
