Edmonds, WA. 98020 attn.: 01a Groset (206) 778-4824 fag: 776-7121 On May 28, 1997, Asbesto-Test personnel conducted a "good faith" asbestos survey (per U.S.E.P.A./A.H.E.&A. guidelines as designated and specified by Puget Sound Air Pollution Control Agency) of the single family dwelling located at 116 3rd Ave. N., Edmonds, Snohomish County, Washington. This survey purpose is to identify any'Asbestos Containing Materials which may be present and will require professional removal prior to renovation or demolition of the structure. BASIC CONSTRUCTION (including siding and roofing types): The house basic construction is wood frame with CAB siding.. The composition roofing and underlying materials were sampled. HEATING AAD VENTILATION SYSTEM: The heating unit is an oil furnace/ hot water system. The oil furnace exterior TSI was sampled, along with the hot water piping TSI. The hot water piping -and relating TSI also runs beneath the house. The chimney flue mortar materials were sampled from the furnace flue. It was not possible to dismantle the furnace to locate or evaluate any additional ACM which maybe inside the unit or internal passeges. It is our opinion any further handling of the heating unit should be coordinated by the project manager and/or abatement contractor. If any additional relating suspect materials are located prior to and/or during any demolition or renovation, any and all additional related suspect materials should be considered to be ACM (Asbestos Containing Material), unless determined to be otherwise by laboratory analysis. ATTIC/WALL INSULATION: Identified insulation is rock wool fiberglass and cellulose materials. PLUMBING SYSTEM: There were no suspect ACM sources located relating to the plumbing system. ELECTRICAL SYSTEM: The electrical wiring insulation was sampled. Any suspect electrical wiring insulation or any suspect TSI (Thermal System Insulation) found in, around, or behind any located fuse or breaker boxes should be considered to be ACM (Asbestos Containing Material), unless determined otherwise by laboratory analysis. INTERIOR CONSTRUCTION, FINISHES, AND FLOORINGS: * The interior construction is wood and plaster. The plaster materials were uniformly sampled. There are no textured ceilings or textured paints. There are no acoustic ceiling tile. Individual homogenous file flooring materials were sampled from each applicable area. *Some sample analyses listed may be a representative analysis of individual and separate samplings and analysis of homogenous materials, as prescribed byA.H.E.RA. protocol. MISC.: The window putty material was sampled. ADDITIONAL BUILDINGS SURVEYED ON PROPERTY: None All material analyses are listed on the following page(s), with a corresponding sample location and description. If asbestos is detected, those samples containing asbestos are listed first, and noted with the initials ACM ("Asbestos Containing Material').
