During re -review of the above noted application, it was found that the following information,
corrections, or clarifications are needed.

5) 11/20/06 Still need. No response given . Still need cross referenced on the site plan. Currentl
the sidewalk and retaining wall are not shown on the site elan. Also. on the site elan
accurately show addition size and location as well as all distances to the property line. Site
plan shows jzaMge addition even with the existing garage but the plans show that it sticks out
about 2.5 feet. Still need Cross reference the retaining wall detail location on the plans as well
as the site plan.

7) 11/20/06 Still need form for Epoxy holdown and anchor bolts. Still need. No response giyeii.
Still need Special inspections are required for the following. Complete the enclosed special
inspection agreement signed by the owner, the general contractor and the special inspector and
submit to the City for approval prior to issuance.
• Retaining wall construction
• Soil bearing verification
• Placement of fill and compaction
• Epoxy holdown and anchor bolt retrofit installation
