During re -review of the above noted application, it was found that the following information,
corrections, or clarifications are needed. Please provide written responses to each comment as to
how it has been addressed (support with code sections where applicable), and where changes can be
found on the plans. Please submit revised plans/documents to Theresa Umbaugh, Permit
Coordinator.
1) Previous Item 3: As previously requested and discussed during meeting on site 1/19/07, provide
evaluation report from structural engineer of record regarding structural integrity of the existing
building/slab to support the proposed truck route through the existing building.
2) The demolition plans submitted under this permit application still show notes regarding Stage 2
and 3 structural demolition. Based on your letter received 12/18/06, the scope of work under
this demolition permit is Stage 1 non-structural demolition only. A separate permit will be
required for Stage 2 and 3 demolition. References to Stage 2 and 3 demolition work have been
crossed out on the plans.
