MANGE OF PROPERTY O NERICGNTRACTt R/LEAD DESIGN PROFESSIONAL L Until the City is, notified in writing otherwise, you are still the Lead Design Professional of record for this project, and Michel Construction is still the Owmer and General Contractor of record. 5/31/07 Response says these have been changed but I was unable to locate any letters addressing this item. It is my understanding after talking to Jeannine Graf, Building Official that a new Architect will be taking over as lead design professional and will be stamping all the plans. We will also need a new Lead Design Professional Statement (see attached #2 for required wording) and new Applicant/Owner Liability Statement from you as the new owner (see attached #11 for required wording). On Sheet A5, there is a now nous regarding pile, location that needs to be created with the structural engineer and rernoved or clarified to give direction to the contractor, 5/31/07 Response said see sheet A5, note has been crossed out but this has not been done. No response needed. Page 2 of 2 APPLICATION CHECKLIST FOR BUILDING PERMIT SUBMITTAL APPOINTMENTS for application submittals must be scheduled with City Officials one week in advance. Only complete submittals, including full review fees, will be accepted for City review. Failure to provide the correct number of copies, plans, or other data will result in the application being rejected for review. VESTING of building permit applications is achieved when the submittal contains each and every document required by this checklist and is substantially complete in all respects. An applicant's rights shall vest when a fully complete building permit application is filed. A fully complete permit application is an application executed by owners of the property, or the duly authorized agent for such owners, contains each and every document required under the terms of these ordinances, and is substantially complete in all respects. All plans, reports and calculations must be prepared and stamped by design professionals licensed by the State of Washington. It is anticipated that minor changes or revisions may be required during any application review process. Such minor revisions or changes shall not keep an application from being deemed complete if a good faith attempt has been made to submit a substantially complete application containing all required components. A fully complete building permit application shall be accompanied by all fees, including, but not limited to, building permit fees and plan check fees. (Ordinance #2769 & ECDC 19.00.015) 1. CRITICAL AREAS DETERMINATION AND/OR HEARING EXAMINER DECISION One Copy A copy of the Critical Areas Determination and Hearing Examiner decision of Criticial Areas Reasonable Use ExceptionNariance, or project specific Planning exemption as noted on the Critical Areas Checklist must be complete at the time of permit submittal, OR, the applicant may request concurrent review of Critical Areas ordinance compliance with the building permit review. Note, building permit review times are extended when concurrent Critical Areas review is requested. 2. LEAD DESIGN PROFESSIONAL DESIGNATION AND STATEMENT Three Copies One design professional shall act as the "lead design professional" for the applicant during the entire permit review process. This licensed professional is responsible for coordinating and incorporating recommendations of other design professionals onto the plans or into the submittal packet. This person shall submit a letter or make a notation on the face of the plans that s/he has reviewed the geotechnical report, understands its' recommendations and have incorporated into the design the established measures to reduce the potential risk of injury or damage from any earth movement predicted in the report. (ECDC 19.10.040) 3. VICINITY MAP Original Within the packet, applicants are provided with the GeoEngineers map of the Landslide area. A signatory block on the map must be filled out by the person who locates the project site on this map. (ECDC 19.10.030) L;temp:bldg/handout/MeadowlMd_checkjlgREV06/07/2005 Page 3 7. Drainage Plan Information Continued NOTE: If the proposed plan requires an easement across land owned by Burlington Northern Railroad, or approval is needed from any other private party, the City shall be provided with a copy of the permit and/or recorded easements. 8. GRADING PLAN Four Copies Refer to Grading, Fill & Excavation handout. Plans shall be prepared and stamped by a State of Washington licensed civil engineer. All geotechnical report recommendations shall be incorporated into the design and detailed on the plans. Use existing grade contours from the topographic map to calculate grading yardage, specifying exact yards of excavation, fill, and grading, as well as excavation yardage for the foundation. Note, grade contours shall be shown at 2 foot intervals for slopes exceeding 15%. Include temporary erosion control measures and permanent slope protection, indicate setbacks from top and toe of slopes. Provide a driveway slope profile and indicate the maximum 4slope. No excavation, clearing or grading for any purpose, including the creation of access to the site is permitted in advance of permit issuance. (ECDC 18.40) Be advised, grading in excess of 250 cubic yards within 200 feet of a shoreline requires a separate Shoreline Permit from the Planning Division. Be advised, grading in excess of 500 cubic yards requires State Environmental Review by submitting a SEPA Checklist. (ECDC 20.55) 9. ARCHITECTURAL AND STRUCTURAL PLANS Thi'ee Copies Refer to New Single Family Residential Permit Submittal handout for general requirements. All architectural and structural plans must be stamped and signed by licensed professionals. Designers work must be reviewed and stamped by State licensed professionals. It shall be the responsibility of the lead design professional to submit, consistent with the findings of the geotechnical report, structural plans which are prepared and stamped by the structural engineer. All architectural plans shall be stamped and signed by the design architect or design engineer, whichever is applicable. The plans shall be consistent with the hazards identified in the Roger Lowe Associates report from 1979 and the 1985 supplement by GeoEngineers, and shall incorporate all geotechnical design requirements or recommendations that are identified in the geotechnical report. Also, provide two stamped and signed copies of structural design calculations. (ECDC 19.10.040) 10. AFFIDAVIT OF NOTICE OF APPLICATION A notice shall be conspicuously posted and maintained on the site indicating that the owner has made application for a permit with the City. The posting shall begin on the day the City accepts the permit application for review and remain until occupancy is granted. Posting signs are available from the City (fee $8.00). An affidavit of posting must be signed by the owner(s), notarized, and submitted to the Building Division on the day the site is posted (form provided by the City). (ECDC 19.10.030C) 11. APPLICANT/OWNER LIABILITY & LANDSLIDE ACKNOWLEDGMENT Three Copies The applicant/owner of the site shall submit a signed and notarized statement to the Building Official that includes the following language: a) That the accuracy of all permit submittal information is warranted by the applicant/owner in a form which relieves the City and its staff from any liability associated with reliance on such permit application submittals. While an application may reference the reports of prior public consultants to the City, all conclusions shall be those of the applicantlowner and her/bis design professionals and, b) That the applicant/owner understands and accepts the risk of developing in an area with potential unstable soils and that they will advise, in writing, any prospective purchasers of the site, or any prospective lessees of structures on the site, of the slide potential of the -T area.
