During review of the subject submittal, it was found that the following information, corrections, or
clarifications would need to be addressed:
1. Street Use permit is not complete.
a. Certificate of Insurance must be active at time of issuance of permit.
b. Acord application included in your resubmittal does not include the wording necessary. Please
make sure that when your insurer issues the certificate that it follows the requirements shown in
our sample and explained in our Street Use handout. (See Handout attached) If the exact
wording and coverages as explained are not reflected on your Certificate of Insurance, it will not
be accepted.
Please submit resubmission to your permit coordinator. You may resubmit by fax: 425-771-0221. Please
