The forms are fine. 'rhe only problem we have now is that when I looked over the lnsurance Ceftificate it does
not list the Dty of Edmonds as an Additional Insured. This is required for Use of public property, t don't know who
is handling this, part of the process, but I if the Insurance company could be contacted arid that corrected, a copy
faxed over to me would be sufficient to issue the permit. Our fax number is 425-771-0221. Please let me know if
sorneone plans to fax it over so I can look for it.
