SUBJECT: Site Improvement Maintenance Bond, Employee Benefits of America
To Whom It May Concern,-
The
oncern:
The City of Edmonds, Engineering Division, has performed the required two-year
maintenance inspection of the site improvements for the Employee Benefits of America
building. In order for the Site Improvement Maintenance Bond to be released the following
corrections must be made:
1. The high capacity gutter that runs across the driveway entrance is cracked in two places
and must be replaced. The panel from expansion joint to expansion joint must be removed
and replaced. See Photo #1.
2. Utility trench was not compacted well and has sunken below the grade of the road.
Current asphalt must be removed. Compaction test will be required prior to re -asphalting.

RE: Compaction Tests:
The City of Edmonds requires compaction of trenches meet the following requirements.
Backfill shall be placed in horizontal layers no more than 6 inches thick ...Compact each layer to a
minimum 95% of maximum density" Density tests must be certified by Washington State licensed
engineer. A minimum of 1 density test shall be taken within every 200 feet of trench length, or, at each
end and the center if a shorter trench, and at depths of 50 percent of trench depth and at the surface, or as
specified by the Engineer. Certified copies of all test results shall be provided to the City.
Test reports may be emailed or faxed to our office, or they can be given to the Engineering Inspector
at time of final inspection.
