[� REINSPECTION FEE MUST BE PAID PRIOR TO NEXT INSPECTION REQUEST. ❑ STOP WORK UNTIL AUTHORIZED TO CONTINUE BY CITY INSPECTOR Per the Correction Notice emailed (a copy also left on site) November 17th (please see attached), it was required that a temporary asphalt patch was to be placed in line with the existing sidewalk. This was to establish a smooth solid surface 5' in width for pedestrians to travel. As of November 23, 2015 the temporary asphalt has not been placed. At this time the city requires the asphalt walkway be installed by December 4th, 2015. The broken sidewalk at the south side of the site will need to be removed reducing the trip hazard. Please see the attached photo for the limits of which the temporary asphalt is to be placed. Failure to complete the asphalt walkway by the end of day on December 41h, 2015 can result in fines and/or a construction STOP WORK ORDER. -The city inspector will stop by Friday December 41h, 2015 to verify the work has been completed and the sidewalk is open. STOP WORK UNTIL AUTHORIZED TO CONTINUE BY CITYINSPECTOR According to conversation with the lead contractor on site, Thursday November 5th, it was understood that the sidewalk was to be opened by the end of the week of November 91h. If sidewalk was to be constructed within the week, a temporary path of gravel was to be placed until required frontage improvements were constructed or temporary asphalt was to be placed. Please address the following issue. 1. At this time the city requires the sidewalk be re -opened by the end of day Thursday November 19th. A path of temporary asphalt 5' in width shall be placed in line with the existing sidewalk. Any other obstructions, such as sanitary cans, shall be removed from the right of way. -The city inspector will stop by Friday November 201h to verify the work has been completed and the sidewalk is open.
