STOP WORK UNTIL AUTHORIZED TO CONTINUE BY CITY INSPECTOR 1)4/3/2015 - The frontage improvements need to be completed. 6/2/2015 –Sidewalk, curb, gutter and road widening improvements were constructed withoutrequired City inspections. Frontage improvements as constructed are not in compliance with the approved plans and/or City standardsand shall be removed and replaced. The following standarddetails,availableon the City website, are provided for reference. After demolition of the recently constructed improvements, the contractor shall schedule a pre-construction meeting with the City. In addition, all form work shall be inspected by the City prior to concrete pour. E2.26–Standard driveway entrance E2.8–Curb and gutter E2.13–Concrete sidewalk E8.4–Regulatory signage E8.5–Mailbox layout Additional information pertaining to thefrontage improvement installation has been provided at theend of this correction notice. 2)4/3/2015 – Asbuilts need to be provided. 6/2/2015 –Asbuilts were provided to theCity and are currently being reviewed. 3)4/3/2015 -Cut the sewer cleanout located at the property line/right-of-way line to grade. 6/2/2015 –Correction not addressed. Discussed with contractor on site –need to have at final grade. 4)4/3/2015 - A 12” cast iron lamphole cover with ½” hexbolts needs to be added over the sewer cleanout. 6/2/2015 –Correction not addressed. Discussed with contractoron site –need to have at final grade. th 5)4/3/2015 - Provide the sewer easement for the portion of the 7822 240St SW sewer that crosses onto th 7824 240St SW’s property. 6/2/2015 –Per Scott, easement is existing andis shown on the asbuilt plans. 6)4/3/2015 - Expose the tracer wire at the meter so that we can verify that it is grounded. 6/2/2015 –Correction not addressed. Tracer wire could not be located. 7)4/3/2015 -Clean outall catch basins. 6/2/2015 –Catch basins on site looked okay. CB in ROW needs to be cleaned. Filter sock shall be in place at time of frontage improvement demolition and replacement. 8)4/3/2015 - Cut back the pipes in the catch basin located adjacent to the driveway, such that they are flush to the wall of the catch basin. 6/2/2015 - correction addressed. 9)4/3/2015 - The catch basin located in the back yard (solid lid) needs to have the pipes cut flush to the wall of the catch basin and mudding around the pipe needs to be reestablished. 6/2/2015 –Pipe has been cut flush to the wall as requested, however the mudding is not complete. 10)4/3/2015 - Replace the cap on the observation well. 6/2/2015 – correction addressed. 11)4/3/2015 - Add a cover/box over the observation. 6/2/2015 – correction addressed. 12)4/3/2015 - Add a cover/box over the cleanout (located at the SE corner) or bury the cleanout a few inches below finished grade so that the pipe does not get damaged during maintenance of the yard. 6/2/2015 – correction addressed. 13)4/3/2015 - Need someone on site to complete a water test on the storm tightline to verify the water discharges to the stormwater management system. 6/2/2015 – correction addressed. 14)4/3/2015 - Need access to the sewer grinder pump to verify that the pump is functioning properly. 6/2/2015 – correction addressed. 15)4/3/2015 - The City’s sewer department will complete an inspection on the sewer lateral and the channeling of the manhole next week. We will follow up if there are additional comments related to that inspection. 6/2/2015 –An inside drop was installed in thesewer manhole in theCity right-of-way, but does not comply with City standards. Please refer to standard detail E6.5 (attached) for drop bowl requirements. Instead of channeling themanhole, it would be acceptable to direct flows via pipe to theedge of the channel.Flows shall not be released on the shelf. Page 2of 6 Information/observations related to the installation of the frontage improvements: 1)Sidewalk cross slope shall not exceed 2% per ADA standards. Several sections are 3% and greater. 2)Driveway ramp shall not exceed 8.3% slope – the built condition is over 10% slope 3)Signs and mailboxes shall not be installed in the street and will need to be placed in the sidewalk per City standards. Both have been built into the road pavement section. Page 3of 6 4)Curb and gutter shall be placed at an elevation that provides for consistent slope of the road from the crown towards the gutter. Stormwater runoff should flow from the crown of the road towards the gutter pan. In the built condition, the curb and gutter is higher than the road grade, which does not meet City standards. 5)Sign post and mailboxes shall be located in the sidewalk, per City standards, and not in the road section. Page 4of 6 6)A2:1 slope shall be providedat theback of sidewalk. Several areas do not meet this slope. 7)Asphalt transition ramp ramp shall not extend into the road section.
