Frontage improvements as constructed are not in compliance with the approved plans and/or City standards and shall be removed and replaced. The following standard details, available on the City website, are provided for reference. After demolition of the recently constructed improvements, the contractor shall schedule a pre-construction meeting with the City. In addition, all form work shall be inspected by the City prior to concrete pour. E2.26 –Standard driveway entrance E2.8 –Curb and gutter E2.13 –Concrete sidewalk E8.4 –Regulatory signage E8.5 –Mailbox layout Additional information pertaining to the frontage improvement installation has been provided at the end of this correction notice. 7/28/15 – a)8/11/2015 – OK The East asphalt ramp does not meet ADA standards. Remove and replace the asphalt ramp such that there is a 4’ landing and ramp. b)8/11/2015 – OK Fog line shall be restriped c)8/11/2015 – Not complete Provide a 2:1 slope at the backedgeof the sidewalk 2)Asbuilts - OK 3)4/3/2015 - Cut the sewer cleanout located at the property line/right-of-way line to grade. 6/2/2015 –Correction not addressed. Discussed with contractor on site –need to have at final grade. 7/28/15 –The cast iron lamphole cover was installed; however, the plastic lid on top of the cleanout cannot be openedat this time. Remove the dirt from the top of the lid and replace the plastic irrigation lid with the standard PVC cap(gasket removed). 8/11/2015 -OK 4)OK 5)OK 6)4/3/2015 - Expose the tracer wire at the meter so that we can verify that it is grounded. 6/2/2015 –Correction not addressed. Tracer wire could not be located. 7/28/15 –Not Addressed. 8/11/2015 –Not complete. Tracer wire present, but not grounded.Please strip the tracer wire and attach to the meter to provide a circuit. 7)4/3/2015 - Clean out all catch basins. 6/2/2015 –Catch basins on site looked okay. CB in ROW needs to be cleaned. Filter sock shall be in place at time of frontage improvement demolition and replacement. 7/28/15 –Catch basin in the ROW has not been cleaned out. 8/11/2015 –OK 8)OK 9)OK 10)OK 11)OK 12)OK 13)OK 14)OK 15)4/3/2015 - The City’s sewer department will complete an inspection on the sewer lateral and the channeling of the manhole next week. We will follow up if there are additional comments related to that inspection. 6/2/2015 –An inside drop was installed inthe sewer manhole in the City right-of-way, but does not comply with City standards. Please refer to standard detail E6.5 (attached) for drop bowl requirements. Instead of channeling the manhole, it would be acceptable to direct flows via pipe to the edge of the channel. Flows shall not be released on the shelf. 7/28/15 –Not addressed. 8/11/2015-Not addressed. Drop bowl is required for a drop inlet, consistent with City standard detail E6.5, but has not been installed. Also, flows shall be directed to edge of sewer flow channel, but pipe shall not extend beyond side of channel. Page 2of 4 16)7/28/15 –A drainage swale with drain rock (from CB1 to CB2) was required as an overflow path per the approved plans. However, as shown in the photo below, this area was covered with crushed rock with fines. Please correct to be consistent with the approved plans. 08/11/2015 –Applicants response to the City is as follows: Rock wastemporarily placed over filter fabric over the top of the catch basin in order to facilitate better access to the rear yard for delivery of new landscaping plantsthat is underway.During this dry season, the additional overflow capacity of the second catch basin was not deemed necessary.Since this was re-inspected by the City during the last inspection and we were told it was approved, we expected that we could then place this temporary rock over the CB so that other work could be more easily accomplished.We never expected that the City inspectors would re-inspect every aspect of the property every time they come out.It was our understanding that the City had no further requests or inspection needs associated with the improvements adjacent to the house and that the inspector's focus would be on the frontage improvements. In order to address your concern, the catch basin has been re-exposed andthe rock that was temporarily placed over it has been removed. The stormwatermanagement system shall be maintained as originally approved and shall not be modified to be covered with rock as shown in the following photos.As noted, the basin was uncovered for the latest inspection. Please revise drainage swale to be consistent with the approved planswhich state “Swale to be 6” +/-with 3:1 maximum side slopes and 1% minimum flow path slope. Place 2”minus washed rock, 3’ wide, over mirafi 140-N fabric”. Please also note, additional impervious surface area shall not be added to the site. Page 3of 4 17)7/28/15 –At the close of a project the number of inspections performed is reviewed against the number of inspections paid for. More inspections were performed on this project than what were paid for at the time of permit issuance. Prior to project final please pay the outstanding balance of $360. This includes 3 additional inspections on the right-of-way permit and 3 additional inspections on the building permit. 8/11/2015 –Applicant has acknowledged.
