Underground 300 Gallon Residential Heating Oil Tank at the above
address. The tank was plumpledfree of aff hazardous materials prior f -o
being foam, filled with a 2 way polyurethane product provided by K& M
Foom and Specialty Coo tings, UC The, Ventilotion and Fill Pipe were
properly dismontled at the time.of the decommlissioning. The Fire
Department Permit & Tank Specification Sheet are attached.
