Please resubmit 3 sets of revised drawings to a Development Services Permit Coordinator. General 1. Since there is a change of use that includes office space and an extra restaurant, a traffic impact analysis will be required and if new pm peak trips exceed 25, at traffic study will be required. 2. Include the previously approved traffic control and truck route plan (under the demo permit) with this permit. 3. The plumbing plan for this site will need to be submitted and looked at before I can sign off on the subject permit. I need to verify that there aren't any conflicts between the plumbing plan and the civil drawings. 4. An encroachment permit will be required for the awnings. If the planter boxes are affixed to the wall, they too need to be included in the encroachment permit. If the boxes are movable, then a street use permit will be required. 5. A sewer connection fee will be assessed on the 2°`'restaurant. Sheet Cl 6. Show the location of the pedestrian tunnel. Remember this must be in place by 1 May 07. 7. Indicate the location of the bus stop and the curb to be painted for the bus stop. 8. Show the location of any telephone, power and gas locations to the bldg. 9. Indicate on the drawing to replace the existing curb cut(s) and sidewalk on Dayton street with standard curb/gutter and new sidewalk. The sidewalk width shall be 10' or to the edge of the right-of-way, whichever is greater and to match existing. 10. Provide a cross section of the new entrance on the n.e. side and the existing sidewalk. How will the new barrier free entrance match up to the existing sloping sidewalk? 11. Add a note stating the new existing stamped concrete crosswalk shall not be cut or damaged. Replace the tree grates removed as a result of comment 8 above and replace with ADA compliant grates. Minimum size shall be 3' x 3' to match existing. 13. A DCDA is required and shall be placed on private property at the property line in the area where the existing concrete ramp is located. Installation shall be per City Standard details. The only other acceptable location for the dcda would be on the interior north wall of the building on Dayton Street and the FDC and PIV would have to be mounted on the side of the exterior wall where the ramp is located and on private property. A remote reader would also have to be placed on the exterior face of the north wall 4.5' above sidewalk grade. In either case, per our details, a 3 -valve cluster will be required at the water main. 14. Indicate a tapping tee and valve for the new 3' domestic. 15. Show the location of the required RPBA with hot box (must be on private property) and premise isolation on the drawing. 16. Show and note requirement for cb protection. 17. Proposed ramp shown will not work at this corner. Provide enlarged and scaled detail of new curb ramps at the corner. 18. The storm line from the building cannot tap into the city storm pipe. Connection must be made into the type 11 cb #5234. 19. If steps are going to be installed in the catch basin at the nw. bldg entrance, polypropylene steps need to be installed. Show the direction of flow of 8" pipe. Show the other pipe penetrations and invert elevations into this cb. 20. Clarify the size of the storm pipe used to connect the private cb to the city's type 11. Two different sizes are noted. 21. Provide cross section of curb ramp area and restaurant entrance to make sure the proper slopes are attainable. 22. The doors at the nw corner of the building appear to encroach into the right of way in the open position. The doors may not encroach into the city right of way. 23. Show the location of the existing sewer lateral that goes from the city ssmh to the bldg corner then down the sidewalk on 5d' ave to the angle point near the s.w. bldg corner where it then goes to the adjacent bldg. This sewer line is in bad shape and was identified to be capped at the main under the demo permit. The adjacent bldg sewer needs to connect to the proposed 8" line shown on the drawing. Indicate the bad sewer line is to be cut and capped at the property line and plugged at the main. 24. Show the location of the sewer and water service lines that will go to the new public bathroom. 25. Will the sewer line from the new public bathroom also connect to the 8" or will it remain separate as indicated on the bathroom permit drawings? If it is to connect, show the sewer cleanout in the correct postion.The drawings need to clearly indicate the relationship between the new bathroom utility connections and the utility connections under this permit application. 26. A sewer manhole will be required to connect the 8" lateral to the existing 10" sewer main. Show the manhole and provide required invert elevations. The sewer cleanout for this line must be in line and at the property line and not as shown on the drawings. Sheet A1.0 indicates the potential for a 3rd floor for multi -family. Was this taken into consideration when sizing the sewer lateral to 8"? 27. Show construction staging area. 28. Show the location of the construction fencing. 29. If the irrigation lines end up in the city right- of-way, the existing sidewalks along 5th Ave and along Dayton street will have to be replaced and at the appropriate 2% slope. The sidewalk width shall go up to the building as it does now. 31. Show upper parking lot striping on the civil drawings as well. 32. Per Federal requirements, and a recent citizen complaint, new curb ramps with truncated domes will be required at the upper entrance off Maple St. FYI: According to our previous Traffic Engineer, the owner is at risk for litigation relating to Federal Accessibility Standards from the Accessibility Advocacy group. 33. General Notes a) note 3: delete " be returned to the equivalent of their preconstruction condition" and add "be replaced" and add"standards to the end of the sentence. 34. General note #6 refers to new paving. Does this mean trench patch work?
