The site takes access from 760 Ave. W. approximately 450' north of the intersection with 244'h St. SW. The site address is: 24223 76b Ave. W. — Edmonds, WA 98026 A. ENVIRONMENTAL ELEMENTS 1. Earth a. What types of emissions to the air would result from the proposal (i.e., dust, automobile odors, industrial wood smoke) during construction and when the project is completed? Proposed measures to reduce or control emissions or other impacts to air, if any: Water will be sprayed on the site to combat dust if necessary. 3. Water a. Surface 1) Is there any surface water body on or in the immediate vicinity of the site (including year-round and seasonal streams, saltwater, lakes, ponds, wetlands)? If yes, describe type and provide names_ If appropriate, state what stream or river it flows into. Yes. Lake Ballinger lies on the. eastern boundary of this project. 2) Will the project require any work over, in, or adjacent to (within 200 feet) the described waters? If yes, please describe and attach available plans. Yes. Clearing, Grading and Building will occur within 200' of the lake. Please refer to Wetland Report & Site Plan (sheet P-1). 3) Estimate the amount of fill and dredge material that would be placed in or removed from surface water or wetlands and indicate the area of the site that would be affected. It appears that a portion of the site is encumbered by zone "A" on the FEMA flood maps. Zone "A" are areas where base flood elevations have not yet been determined. 6) Does the proposal involve any discharges of waste materials to surface waters? If so, describe the type of waste and anticipated volume of discharge. No proposal of discharge of waste materials. Site will be served by sanitary sewer. b. The parcels to the north and south are in residential use with single-family residences on them. To the west lies 76th Ave. W. To the east lies Lake Ballinger. b. Has the site been used for agriculture? If so, describe. Not to our knowledge. c. Describe any structures on the site. Three single-family residences and outbuildings. d. Will any structures be demolished? If so, what? Three existing single-family residences and outbuildings. e. What is the current zoning classification of the site? RS -8 & RSW-12. f. What is the current comprehensive plan designation of the site? Single Family Urban 1 & Single Family Resource. g. If applicable, what is the current shoreline master program designation of the site? Suburban Residential IV h. Has any part of the site been classified as an "environmentally sensitive: area? If so, specify_ Yes. One Category IV Wetland ties on the eastern side of the site, abutting Lake Ballinger. The wetland has a DOE rating score of 25. 7 TO BE COMPLETED BY APPLICANT . 9. Housing i. Yes, Community Transit Routes: 130 Runs on 76"Ave. W. & 416 & 870 Run on SR 140 to the south. c. How many parking spaces would the completed project nave? How many would the project eliminate? The completed project will have 32 parking spaces. Assuming 4 spaces per SFR, 12 spaces will be eliminated. d. Will the proposal require any new roads or streets, or improvements to existing roads or streets, not including driveways? If so, generally describe (indicate whether public or private). Yes, a new public road with a hammerhead turnaround will be constructed on the north side of the site. e.
