The application for the Monopole extension will expire March 28, 2017. We still need contractor information with
current City and State license numbers before we can issue. This application can be extended an additional 6 months

upon request. If not issued by March 28, 2017 or we do not hear from you to extend this application, this application
will expire. In order to do work a new application, plans and fees will be required.
