The following items shall be completed/corrected/submitted prior to final
acceptance.
1. Replace curb and gutter on the south side of 216th removed for water line
installation.
2. At same location, replace curb at edge of parking lot. Sawcut additional
existing curb to remove area damaged during removal.
3. Pedestrian crossing at northwest corner of property connecting the
existing west sidewalk to the new pedestrian island is shown as concrete
in the plans to delineate the crosswalk from the asphalt. The new
crosswalk area has been paved.
4. Complete all pavement and curb striping, lettering and painting.
5. Install parking stall wheel stops.
6. Install all signage.
7. Install rip rap in bio swales.
8. Southeast parking stall does not meet required sizing.
9. Some areas of asphalt need additional sealant along sawcuts.
Comments Provided „to_Gregco: (see attached correction notice)
1. Vactor out storm lines and middle manhole at south end of detention
system.

2. Mudding needed on SDMH 02 and CB 01.
Provided follow-up on the following items to Andy.
3. Fire Hydrant base -flange to concrete pad.
F/U: Remove material at base of hydrant in Pavillion parking lot. Other
hydrant installations approved.
4. Side Sewer clean outs have the wrong lids. CoE standard requires 12" lid
with %2" hexbolts.
F/U: Size and model of sewer clean out lamphole cover is called out on
civil sheet C-511 of the approved plans. Detail also requires a 14" x 14" x
6" leveling collar. Please replace lids and install concrete collar.
The following documents are required prior to final:
1. Civil Asbuilts, one electronic copy and one paper copy.
2. Maintenance Bond equal to 15% of the Perfomance Bond $ 298,565.00
3. Truck tickets for the composted amended swale soil.
Remaining Inspections:
The water and sewer division will be on site tomorrow, 6/10, to make a final
check.
The stormwater division will be on site at some point to gps the detention
system. This is not a requirement for final approval.
All other public works divisions have provided approval.
