An environmental impact statement (EIS) must be prepared for all proposals with probable significant adverse impacts on the quality of the environment. The purpose of this checklist is to provide information to help you and the agency identify impacts from your proposal (and to reduce or avoid impacts from the proposal, if it can be done) and to help the agency decide whether an EIS is required. A. BACKGROUND th 1. Name of proposed project, if applicable: 215 Street Rezone. 2. Name of applicant: City of Edmonds. 3. Address and phone number of applicant and contact person: Gina Coccia, Associate Planner – 425.771.0220 x 1778. th City of Edmonds Development Services Department – 121 5 Avenue North, Edmonds WA 98020. 4. The proposed use will be single family residential. The area proposed to be rezoned is approximately 4.5 acres and is th comprised of those properties along the north and south sides of 215 Street SW that are also designated “Single Family – Urban 1” on the Comprehensive Plan map. 12.
