BEFORE A, ]PERMIT CAN BE ISSUED: The owner atld eeatractor and special inspector shall complete
this agreement and the attached sitrrutnrsl tast(s) and it speodons bchedule induding the regWred
acknowledgements.
APPROVAL OF SPECIAL IKSPECTORS: Each special inspector sbpil be approved by the Building
Official prior to performing any duties air Inspections. Each special inspector shall submit Statement of
Quohneations to the Building D(lieiol for review. Special inspectors shall dlspinx identification *ben
performing special inspections on site. Special inspection and testing dead men the minimum requirements

1. Observe Work
The special inspector shall obsRrve the site work for conformian ce with the approved (stamped) places

3. Complete Daily Reports
Each special Inspector shut) complete and sign both the special Inspection record and flee daily report
form for each day's inspection. These recordx shall remain at the job!rke with the contractor for
review by the City Building Inspector.
4. Fumish Weekly Reports
The special inspector or inspection agency shall furnish The City with weekly reports of tests and
Inspections. The project englaeer or architect, and others nes designated shall also be copied an
reports. Weekly reports must include the following:
• De.4criptioa of daily inspections send tests evade with applicable locations
• List of real uou-ton orming items
• Rrport on status of non-omforming itcm s (resolved or unresolvFd)

conformance items.
