This is to document that the special inspection of the epoxy anchors per IBC 1704 should be
deemed complete. I understand that the special inspection was not completed by Otto Rosenau as
indicated in the permit documents. Instead I have verified that the anchors were installed as
required in ICC ESR-2508. The holes were properly drilled, cleaned out, and the epoxy was
installed at an acceptable temperature. Upon curing, the nuts were tightened and there was no
evidence of tensile failure of the epoxy anchors. The installation is complete.
Please kindly call with any questions or concerns about the installation of the anchors for this
project.
