RESOLUTION NO.
WHEREAS the City of Edmonds has had a group
insurance plan for the employees of said city with the
United Pacific Insurance Company which coverage was cancelled
effective September 30, 1964 by action of the City Council of
the said City, and
WHEREAS effective October 1, 1964 similar coverage
was adopted by action of the City Council of the City of
Edmonds underwritten by the New York Life Insurance Company,
now, therefore,
THE CITY COUNCIL OF THE CITY OF EDMONDS RESOLVES
that said group insurance program is to be underwritten by
the New York Life Insurance Company, and includes Life,
Accidental Death and Dismemberment, Weekly Indemnity, and
Major Medical Insurance for employees' dependents, and it is
FURTHER RESOLVED that the City of Edmonds' contri-
bution will be the employees' cost of the plan.

this 17th day of November, 1964.
